The collaborative study between CU Anschutz, the Allen Institute (搜索), UC San Diego (搜索), and Benaroya Research Institute followed individuals with anti-CCP antibodies (搜索), a known biomarker for rheumatoid arthritis (搜索) risk. Dr. Kevin Deane, director of the Autoimmune Disease Prevention Center at CU Anschutz and one of the study's senior authors, explained that participants with this specific antibody type had about a 30% risk of developing rheumatoid arthritis.
Pre-Clinical Immune System Changes Detected
The research uncovered previously unknown biological changes occurring before the first swollen joints that define full-blown rheumatoid arthritis (搜索). "This study builds on years of work identifying and tracking people at risk for RA," said Deane. "By applying cutting-edge technologies, we were able to uncover how the immune system shifts long before the first swollen joints that define the presence of full-blown RA appear."
The study identified several key immune system alterations in at-risk individuals:
Systematic Inflammation: Even before symptoms and joint swelling appeared, at-risk individuals showed body-wide inflammation similar to what's seen in active rheumatoid arthritis (搜索) but without joint inflammation.
Immune Cell Dysfunction: B cells (搜索), which normally protect the body, were promoting inflammation instead. T helper cells (搜索), especially a type linked to harmful antibodies, were overactive and attacking healthy tissue.
Cellular Reprogramming: Even "naive" T cells that hadn't yet encountered threats showed changes in gene activity that primed them for autoimmune behavior.
Joint-Targeting Signals: Monocytes (搜索) in the bloodstream produced inflammatory molecules and resembled cells found in rheumatoid arthritis (搜索)-affected joints, suggesting the immune system was already preparing to target joint tissue.
Clinical Implications for Early Detection
About 1% to 2% of people have the anti-CCP antibody, indicating they could be at high risk and might benefit from further monitoring, according to Deane. The findings offer new early-warning signs, biomarkers, and immune signatures that could help healthcare providers identify which at-risk individuals are most likely to develop rheumatoid arthritis (搜索).
"This gives us multiple biological targets to explore for prevention," said Deane. "It also helps us better predict who will actually develop RA, because not everyone with risk markers like anti-CCP will get the disease. This new data will help accelerate research into how RA starts and how we can stop it."
A second study currently enrolling on the Anschutz campus will follow people with the particular antibody for five years to test preventive measures. People who might be at higher risk because a relative has rheumatoid arthritis (搜索) can get tested for the antibody to see if they're eligible for monitoring.
Hydroxychloroquine Trial Results
In a separate study published in The Arthritis and Rheumatology Journal, CU Anschutz researchers found that hydroxychloroquine, a drug commonly used to treat rheumatoid arthritis (搜索), did not prevent the disease in people at high risk. The trial called 'StopRA' followed 144 participants with elevated anti-CCP antibodies (搜索) over three years, with participants in both the hydroxychloroquine and placebo groups developing rheumatoid arthritis in similar numbers.
"This helps us rule out a treatment many healthcare providers and researchers hoped might delay RA, and may have been using already," said Deane. "Furthermore, the findings from StopRA trial, as well as forthcoming mechanistic studies which were done as part of the trial, will help us identify more effective prevention strategies."
Current Prevention and Future Directions
Currently, the only prevention options for people at risk of rheumatoid arthritis (搜索) are lifestyle changes, such as quitting smoking, exercising, and eating a healthy diet. However, with more understanding of the underlying immune system changes, researchers can start testing whether other treatments could prevent or delay rheumatoid arthritis, or lead to less-severe symptoms when it eventually develops.
About 70% of people with rheumatoid arthritis (搜索) need more than one drug to get the condition under control. If treatment can begin before joint damage occurs, patients may be able to take fewer drugs and avoid some side effects. The approach has precedent: children at risk for type 1 diabetes (搜索) can now take medication to delay damage to their pancreases.
The research team continues building a network of at-risk individuals through studies like StopRA and the Allen project. "Combining these efforts will help validate findings and move the field closer to the point where we implement prevention of RA, and ultimately other autoimmune diseases, in routine health care," said Deane.
