Study Design and Patient Population
The researchers conducted a population-based target trial emulation study analyzing over 18,000 patients diagnosed with both gout (搜索) and type 2 diabetes (搜索). All participants filled new prescriptions for glucose-lowering treatments—either SGLT2i, DPP-4i, or glucagon-like peptide 1 receptor (搜索) agonists (GLP1-RA (搜索))—and were not currently using urate-lowering therapies (ULTs).
The study team used statistical techniques to balance baseline characteristics between treatment groups, including demographics and number of recent gout (搜索) flares, effectively mimicking randomization. Patients were followed over time to compare the probability of starting ULTs, rates of gout flares, and prescriptions for gout flare treatments and diuretics.

"Conventional urate-lowering therapies are recommended for flare prevention, but they don't help with the CKM comorbidities, making it necessary for patients with co-occurring conditions to take medications in different classes to treat both," McCormick explained. "This makes treatment decisions more complex, while increasing pill burden for patients and the potential for drug interactions."
Key Findings
Beyond the 38% reduction in allopurinol initiation probability, patients using SGLT2i demonstrated several additional benefits:
- Fewer prescriptions for NSAIDs, colchicine, and high-dose glucocorticoids
- Reduced frequency of gout flares
- Fewer prescriptions for diuretic medications, particularly loop diuretics

Future Research Directions
The study opens new avenues for investigating polypharmacy in gout (搜索) patients. McCormick highlighted that polypharmacy, typically defined as regularly taking at least five medications, "hasn't been well studied in gout, so that's an avenue I'd like to pursue further in the future."
The research suggests that SGLT2 inhibitors (搜索) may offer a dual therapeutic benefit, addressing both glucose control and gout (搜索) management while potentially streamlining care and reducing overall medication burden for this complex patient population.
