Simcha Therapeutics (搜索) announced promising preclinical results demonstrating that their decoy-resistant IL-18 (搜索) variant, ST-067, significantly enhances the anti-tumor efficacy of bispecific T-cell engagers (搜索) (BiTEs) across multiple cancer types. The findings were presented at the European Society for Medical Oncology (ESMO) Congress in a presentation titled "Decoy-resistant IL-18 combination with bispecific T-cell engager enhances anti-tumor efficacy."
Enhanced Tumor Growth Inhibition Across Cancer Types
The preclinical data showed that adding ST-067 to BiTE treatment enhanced tumor growth inhibition without observed weight loss, compared to treatment with BiTEs alone. The combination therapy demonstrated positive results across models of both solid and hematologic malignancies, including melanoma (搜索) and humanized models of colorectal cancer (搜索) and B-cell acute lymphoblastic leukemia (搜索).
Notably, ST-067 demonstrated potential to induce immunological memory that inhibited tumor growth upon re-challenge, compared with an untreated control group. This finding suggests the therapy may provide durable anti-tumor responses beyond the initial treatment period.
First-in-Class Decoy-Resistant IL-18 Technology
ST-067 represents the first decoy-resistant variant of IL-18 (搜索), specifically designed to be impervious to the decoy receptor IL-18BP, which prevents IL-18 from binding to its receptor. This engineering breakthrough allows the cytokine to maintain its therapeutic activity despite the presence of natural inhibitory mechanisms that typically limit IL-18 efficacy.

Clinical Development and Future Collaborations
As the most clinically advanced engineered IL-18 (搜索) agonist, ST-067 is currently being evaluated in multiple clinical studies, both as monotherapy post hematopoietic stem cell transplant and in combination with CAR-T cell therapy. The company has established a partnership with Janssen Biotech (搜索) for development of CAR-T cell therapies armored with decoy-resistant IL-18 (搜索).
Ravindran indicated that the company plans to advance collaborations to evaluate ST-067 in combination with bispecific therapies while continuing development efforts integrating ST-067 with additional forms of T-cell mediated therapeutics. This approach positions ST-067 as a potential platform technology that could enhance multiple immunotherapy modalities.
