Singapore Launches Phase III of National Precision Medicine Programme to Enroll 400,000 Patients

Singapore has launched the third phase of its National Precision Medicine (NPM) programme, marking a significant transition from research-focused genomic studies to population-scale clinical implementation. The initiative, running from 2025 to 2031, aims to enroll 400,000 to 450,000 patients directly from healthcare settings across the nation's three public healthcare clusters.
Precision Health Research Singapore (PRECISE (搜索)) signed a Memorandum of Understanding with NHG Health (搜索), National University Health System (NUHS), and SingHealth to facilitate this ambitious undertaking. The collaboration represents a commitment to enroll approximately 10% of Singapore's local resident population as study participants over the next five years.
Shifting Focus from Healthy Cohorts to Clinical Populations
Phase III represents a fundamental shift in approach from previous phases of the programme. Professor Patrick Tan, Executive Director of PRECISE (搜索), explained that "In Phase II, we studied healthy cohorts to contribute to population health research. In Phase III, we will work with individuals receiving medical treatment and clinical services, including those with specific disease conditions."
This transition will enable researchers to study how genetic, lifestyle, and environmental factors interact in real-world clinical settings, deepening understanding of disease progression and enabling tailored treatment approaches for individual patients. The large sample size will particularly benefit the detection of rare diseases and less common genetic variants in the population.
Building on Previous Success
The NPM programme began in 2017 with Phase I creating a pilot database of 10,000 Singaporean genomes (SG10K), followed by Phase II in 2021 mapping DNA from 100,000 healthy Singapore residents (SG100K). Research under NPM has already advanced national health initiatives, notably through the creation of genomic assessment centres for earlier diagnosis and management of familial hypercholesterolemia (搜索) (FH), a genetic condition that significantly increases heart disease (搜索) risk.
More than 200 individuals have consented to genetic testing under the FH programme since its launch in June, according to Health Minister Ong Ye Kung. PRECISE (搜索) has also conducted five clinical pilots involving over 6,000 patients exploring how genetic information can guide drug use and assess disease risk.
Financial Support and Insurance Protection
The Ministry of Health is providing substantial financial support to ensure accessibility of genetic testing. Approved tests will be subsidized and payable with MediSave, with FH testing having an out-of-pocket cost of less than $100 after subsidies. Seniors aged 60 and above may use Flexi-MediSave to completely cover their costs, while cholesterol-lowering medications for FH-positive patients, including specific statins (搜索) and PCSK-9 inhibitors (搜索), are also subsidized.
To address public concerns about genetic discrimination, MOH introduced a moratorium in 2021 preventing insurers from using genetic test results in underwriting. This protection has been extended to cover all FH diagnostic genetic results under the national programme, with both diagnostic and predictive genetic tests coming under the moratorium.
Strategic Vision for Asian Precision Medicine
Professor John Chambers, Chief Scientific Officer at PRECISE (搜索), emphasized Singapore's unique positioning: "Singapore is uniquely positioned to lead precision medicine in Asia. Our multi-ancestry population, strong public healthcare and research infrastructure, and ability to integrate health and research data make it possible to generate insights that are clinically relevant for the Chinese, Malay, Indian and other Asian communities."
Health Minister Ong Ye Kung articulated the transformative vision: "We must move from a system that reacts to illness to one that predicts and prevents it – and ultimately preserves health. When the deep biological insights of genomics are combined with the predictive power of AI plus the policies of preventive care and population health, we transform the healthcare system in a very fundamental way."
Clinical Implementation and Future Applications
Professor Tai E Shyong, Chief Medical Officer at PRECISE (搜索), noted that "DNA (genetic) testing is already making a difference in selected clinical settings—helping doctors detect conditions earlier, explore targeted therapies, and refine care approaches." The programme anticipates that as more gene-disease pairs are identified, greater awareness of genetic predispositions could encourage more preventive and timely healthcare interventions.
Financial support extends beyond diagnostic testing to include approved cell, tissue and gene therapy products (CTGTPs), specifically Kymriah for B-cell acute lymphoblastic leukaemia (搜索) treatment and Yescarta for large or high-grade B-cell lymphoma.
Clinical Associate Professor Tan Ee Shien, Chief Innovation Officer at PRECISE (搜索), emphasized the systematic approach: "Together with our hospital partners and MOH, we are working systematically to assess clinical utility, population health impact, system readiness, and patient perspectives in more disease areas so as to guide these innovations through the appropriate pathways for clinical adoption."
The programme represents a cornerstone of Singapore's Research, Innovation and Enterprise (RIE) 2025 plan and is recognized by the Ministry of Health as a key enabler of future-ready healthcare, positioning the nation at the forefront of precision medicine implementation in Asia.
