Sparrow Pharmaceuticals Initiates Phase 2b Trial of Clofutriben for Difficult-to-Control Type 2 Diabetes

Novel Mechanism Targets Cortisol-Driven Metabolic Dysfunction
Clofutriben is a selective inhibitor of 11β-hydroxysteroid dehydrogenase type-1 (搜索) (HSD-1 (搜索)) that reduces cortisol production at the intracellular level in key metabolic tissues including liver, fat, pancreas, muscle, bone, and brain. This mechanism offers a fundamentally different treatment approach compared to existing diabetes therapies by addressing excess cortisol, a well-understood driver of metabolic dysfunction (搜索) that causes hyperglycemia, hypertension, weight gain, myopathy, dyslipidemia, and osteopenia.

Two-Part Trial Design Emphasizes Precision Approach
CAPTAIN-T2D employs a two-part design to identify and treat the appropriate patient population. In Part 1, patients with poorly controlled type 2 diabetes (搜索) despite multiple antidiabetic medications will be screened for elevated cortisol using a simple overnight dexamethasone suppression test. Eligible participants with elevated cortisol will advance to Part 2, where they will be randomized to receive one of several doses of clofutriben or placebo in a dose-ranging study.
The primary endpoint is change in hemoglobin A1c (HbA1c) at 24 weeks. Exploratory endpoints will assess changes in other cardiometabolic parameters that can be impacted by elevated cortisol, including body weight, blood pressure, cholesterol, and bone formation markers.
Addressing Significant Unmet Medical Need
The trial targets a substantial patient population with high unmet needs. Approximately half of patients with type 2 diabetes (搜索) today are not adequately controlled, and up to half of patients with difficult-to-control type 2 diabetes have elevated cortisol levels associated with increased comorbidities contributing to cardiometabolic risk. Recent research has demonstrated that these patients can be easily identified with a simple, inexpensive, and widely available test, and that a cortisol-directed agent can improve glycemic control when polypharmacy with current agents has failed.

Promising Clinical Evidence from Previous Studies
In previous Phase 2 clinical trials, clofutriben demonstrated improved glycemic control in patients with type 2 diabetes (搜索), including individuals with both type 2 diabetes and elevated cortisol due to ACTH-dependent Cushing's syndrome (搜索). The compound maintained a favorable safety and tolerability profile, requires no dose titration, and has shown no evidence of causing adrenal insufficiency.
Beyond glycemic improvements, clofutriben has shown evidence for benefits in individual patients across multiple related metabolic parameters, including body weight and composition, lipids, blood pressure, and bone metabolism markers, as well as sleep and quality of life improvements.
The once-daily oral medication represents a novel mechanism that complements existing agents by addressing an underlying cause of disease progression and treatment resistance across the spectrum of metabolic dysfunction (搜索), offering potential benefits for the millions of patients who continue to struggle with cardiometabolic diseases despite current therapeutic options.
