Comprehensive Genomic Profiling Reveals Unique Pediatric Tumor Biology
The STREAM Program identified 1,082 somatic driver mutations (搜索) across 308 patients, with 56% consisting of non-SNV events including 238 copy number variants (搜索) (CNVs) and 323 structural variants (搜索) (SVs). Notably, 82% of patients had at least one somatic driver, spanning more than 1,000 genomic events across 400+ genes.
The analysis revealed distinctive characteristics of pediatric tumors compared to adult cancers. Pediatric tumors exhibited a 14-fold lower mutational burden and typically contained only two driver events per patient, highlighting pediatric-specific therapeutic vulnerabilities that differ significantly from adult oncology.
"This national-scale program shows that genome-guided care for children with cancer is no longer a future concept – it is happening right now in clinics," said Jehee Suh, CEO of Inocras (搜索). "STREAM has already delivered clinically actionable findings in more than half of patients, driven in large part by structural and fusion variants, and is uncovering unique aspects of pediatric tumor biology such as low mutational burden and single pathway dependencies."
Clinical Implementation and Real-World Impact
Launched in Korea in 2023, STREAM integrates whole genome sequencing, transcriptome and methylome profiling, and ex-vivo drug screening into routine pediatric care. The program's multi-omic approach has proven effective in real-world clinical settings, with molecular profiling not only mirroring histology but also revealing new genomic clusters that more accurately predicted treatment response.
The 59% of patients who achieved clinically actionable results experienced direct benefits including diagnostic refinement, therapeutic guidance, or clinical trial eligibility. Additionally, 10% of patients carried pathogenic germline variants (搜索), supporting family counseling and early interventions.
Ex-vivo drug testing demonstrated strong concordance with genomic signatures, enabling genome-guided therapy recommendations in real-world tumor boards. These findings provide crucial support for designing future pediatric basket trials based on pathway dependencies rather than traditional tumor morphology.
Advancing Pediatric Precision Medicine

The program is on track to reach 500 patients in the near term as part of a broader 10-year initiative targeting 2,000 patients nationwide.
