Structured Exercise Program Reduces Chemo Brain in Phase 3 Trial

A nationwide Phase 3 trial of 687 cancer patients found that a structured home-based exercise program called EXCAP significantly reduced cognitive impairment and mental fatigue during chemotherapy treatment.
Patients following the exercise prescription maintained their daily walking levels while those without structured exercise reduced their daily steps by 53% during chemotherapy.
The cognitive benefits were most pronounced in patients receiving chemotherapy on 2-week cycles compared to those on 3-4 week treatment schedules.
A structured, home-based exercise program has demonstrated significant efficacy in reducing cancer-related cognitive impairment, commonly known as "chemo brain," according to results from a nationwide Phase 3 clinical trial published in JNCCN—Journal of the National Comprehensive Cancer Network.
The study, led by researchers from the Wilmot Cancer Institute at the University of Rochester, enrolled 687 cancer patients receiving chemotherapy for the first time across 20 community oncology clinics. Up to 75% of cancer patients experience cancer-related cognitive difficulties during treatment, manifesting as brain fog, memory lapses, and trouble managing daily activities such as medications, finances, and household tasks.
EXCAP Program Design and Implementation
The Exercise for Cancer Patients (EXCAP) program was developed in collaboration with the American College of Sports Medicine and designed to provide safe, practical, and personalized exercise during chemotherapy. The prescription includes progressive aerobic walking and resistance band exercises tailored to individual physical capabilities.
"This is a safe and simple exercise prescription that can be an important part of supportive care for anyone going through chemotherapy," said Karen Mustian, PhD, MPH, dean's professor of Surgery, Cancer Control, and associate director of Population Science at Wilmot. "Cancer care providers should educate their patients about home-based options such as walking and resistance band exercises as part of optimal care."
Patients randomized to the exercise group received individualized instructions using a teach-back method to ensure proper technique and were advised to maintain daily records of their steps and exercise time. Blood samples were collected to track inflammatory markers throughout the study period.
Significant Cognitive Protection Observed
The trial revealed striking differences between patients who followed the structured exercise program and those who received standard care without exercise. Prior to chemotherapy, all participants walked an average of 4,000 to 4,500 steps daily. However, patients without a formal exercise prescription reduced their daily steps by 53% during treatment, while those in the exercise group maintained their baseline activity levels.
"It was striking to find that without a structured exercise plan, patients receiving chemotherapy often reduce their daily walking by half and experience notable increases in problems with thinking, memory, and mental fatigue," said Po-Ju Lin, PhD, MPH, RD, research assistant professor and member of Wilmot's Cancer Prevention and Control research program.
Two-Week Treatment Cycle Shows Optimal Benefits
The cognitive benefits were most pronounced among patients receiving chemotherapy on 2-week cycles compared to those on 3-4 week treatment schedules. Patients on 2-week cycles who followed the exercise prescription demonstrated significantly reduced cognitive decline overall (mean difference = 7.0; P = .04), including reduced perceived cognitive impairment (mean difference = 4.1; P = .05) and mental fatigue (mean difference = 4.1; P = .05).
All EXCAP participants reported less mental fatigue (-0.7; P = .02) regardless of their chemotherapy cycle frequency. The study authors suggested that 2-week chemotherapy cycles may represent a "sweet spot" that allows patients sufficient recovery time to continue exercising consistently.
"The patients on two-week cycles of chemotherapy may be getting drugs that have different toxicities and less-severe side effects, which may allow them to remain more active," Mustian explained. "Once a person starts lowering their activity levels, it is more difficult to get back to their baseline activity or maintain it."
Anti-Inflammatory Mechanisms Support Cognitive Function
The research confirmed that exercise creates a systemic anti-inflammatory environment that supports immune system function during cancer treatment. Suppressed inflammatory responses were associated with cancer-related cognitive impairments in all patients receiving chemotherapy (0.2; P < .01), supporting the biological rationale for exercise intervention.
Lin emphasized the importance of non-pharmacologic interventions for managing cognitive impairment during chemotherapy. "Nonpharmacologic interventions, such as exercise, cognitive training, and mindfulness, are important for managing cognitive impairment during chemotherapy because they are safe, accessible, and can often be delivered at low cost or even at home, making them easier for patients to use, compared with expensive or clinic-based treatments."
Clinical Implementation Recommendations
The study authors recommend that cancer care providers incorporate structured, home-based exercise prescriptions into routine chemotherapy care as part of optimal supportive care. They suggest educating patients about the cognitive benefits of exercise, monitoring cognitive and fatigue symptoms throughout treatment, and referring patients to exercise oncology specialists when needed to tailor programs to individual capabilities.
The research was conducted through the University of Rochester/National Cancer Institute Community Oncology Research Program (NCORP) Research Base, a nationwide translational science network designed to conduct clinical trials in community settings. The study enrolled patients between 2009 and 2014, with the majority being women who had undergone surgery and/or had breast cancer, and none had distant metastases.
