Study Finds Strong Link Between Cutaneous Melanoma and Uveal Melanoma Risk

Though cutaneous and uveal melanomas have distinct genetic drivers and progression pathways, both originate from neural crest melanocytes and may be linked in ways not yet fully understood. Studies examining their relationship have historically been limited to small sample sizes due to the rarity of uveal melanoma (搜索), but a recent paper published in Cancers provides larger-scale data.
Using a large, multicenter cohort derived from the TriNetX database, researchers retrospectively analyzed the long-term risk of uveal melanoma (搜索) in two types of skin cancer and found a strong link to cutaneous melanoma (搜索).
Study Design and Cohorts
The researchers created two patient cohorts. Cohort 1 included patients with cutaneous melanoma (搜索) or melanoma in situ (搜索) and no history of keratinocyte carcinoma (搜索). Cohort 2 included patients with keratinocyte carcinoma and no history of cutaneous melanoma or melanoma in situ. Patients with keratinocyte carcinoma (also known as basal or squamous cell carcinoma) served as controls, since the disease shares key characteristics with cutaneous melanoma—though the two are distinct cancers—and to reduce the potential confounding influence of sunlight exposure and access to health care.
Patients with metastatic disease were excluded from the study to avoid potential misidentification of choroidal metastasis as uveal melanoma (搜索). Propensity score matching resulted in two cohorts, each with 52,860 patients.
Key Findings
The study's main outcome measure was the incidence of malignant neoplasms of the choroid or ciliary body over a five-year period, including those recorded before skin cancer diagnoses. The researchers found that uveal melanoma (搜索) occurred in 53 patients with cutaneous melanoma (搜索) (0.1%) and in 13 patients with keratinocyte carcinoma (搜索) (0.02%), demonstrating an odds ratio of 4.08.
Uveal melanoma (搜索) diagnosis preceded cutaneous melanoma (搜索) in 70% of patients with both diagnoses. The researchers reported that this association remained after a sensitivity analysis that excluded patients with documented uveal melanoma before cutaneous melanoma. In their Cancers paper, they explained that this suggests "a shared susceptibility between cutaneous and uveal melanoma of unknown origins," explained only in part by rare genetic predisposition syndromes.
Clinical Implications
The authors emphasized that their findings establish an association, not a causal link between the two cancers. Nevertheless, "given the poor prognosis of metastatic uveal melanoma (搜索), identification of high-risk groups is clinically important," they wrote.
Since uveal melanoma (搜索) screening is not conducted on the general population, the authors advised that eyecare providers managing patients with a history of cutaneous melanoma (搜索) maintain vigilance for potential ocular symptoms and consider dermatology referral for those with malignant uveal findings.
