Study Reveals Critical Gaps in Lyme Disease Follow-Up Care and Diagnostic Testing

The research, published July 10 in Frontiers in Medicine, tracked 253 participants in the Lyme Disease (搜索) Biobank program across Long Island, New York, and Central Wisconsin between 2014 and 2023. The findings highlight critical gaps in both patient care continuity and diagnostic accuracy that may contribute to long-term complications.
Diagnostic Testing Falls Short
The study found that the CDC (搜索)'s recommended standard two-tier testing (STTT) algorithm detected only 23% of Lyme patients presenting with early signs or symptoms. Even more concerning, the blood test identified just 34% of patients who presented with erythema migrans (搜索) rashes larger than five centimeters—a presentation "considered diagnostic for Lyme disease (搜索)," according to the researchers.
These results align with previous research indicating that current blood tests miss up to 70% of early-stage Lyme disease (搜索) cases. Immunoglobulin G seroconversion, which would indicate immune system response to the infection, was observed in only 4% of samples during the study period.
Persistent Symptoms and Limited Follow-Up
The study revealed that 22% of participants continued to experience Lyme disease (搜索) symptoms three months after initial treatment. The most commonly reported ongoing symptoms included joint pain (搜索), fatigue (搜索), and muscle pain (搜索). However, of those patients with persistent symptoms, only 35% followed up with their healthcare provider.
"Accurate, timely detection of Lyme disease (搜索) is critical to preventing long-term complications," said lead researcher Liz Horn, principal investigator of the Bay Area Lyme Foundation (搜索)'s Lyme Disease Biobank program. "Moreover, follow-up from medical professionals with all patients after they complete antibiotic treatment could improve outcomes and reduce the burden of Lyme disease."
Barriers to Continued Care
While the study did not investigate specific reasons for the low follow-up rates, researchers noted that previous studies have identified several barriers to care, including lack of insurance coverage, cost considerations, travel time and distance to obtain care, and limited availability of nearby healthcare services.
Clinical Implications and Recommendations
The research underscores the need for systematic improvements in Lyme disease (搜索) management. The bacterial infection (搜索), spread through tick bites, presents with symptoms including fever, chills, headache, fatigue (搜索), muscle and joint aches, swollen lymph nodes, and the characteristic bullseye rash at the tick bite site.
"Healthcare professionals treating patients with early Lyme disease (搜索) are encouraged to follow-up with their patients, assess them for ongoing symptoms, and consider antibiotic re-treatment as appropriate," the researchers concluded in their report.
The study authors emphasized that "early diagnosis and treatment, with additional follow-up by healthcare providers, has the potential to improve patient outcomes, decrease the percentage of people who progress to persistent Lyme disease (搜索), and reduce the overall burden of Lyme disease."
Research Infrastructure
The Lyme Disease (搜索) Biobank, which provided the foundation for this research, represents a collaborative effort to collect and store human biological samples from Lyme patients. "The Lyme Disease Biobank was visionary in recognizing early on that progress in research was dependent upon having a large number of well-characterized samples from patients with diverse manifestations of Lyme disease available for study," noted Linden Hu, M.D., professor of immunology at Tufts Medical School (搜索) in Boston.
The findings suggest that addressing both diagnostic limitations and care continuity gaps could significantly improve outcomes for the growing number of patients affected by Lyme disease (搜索).
