Published in JAMA Network Open, the research involved 384 participants across 28 outpatient physical therapy clinics in six Midwest health care systems.
Significant Pain Reduction Achieved
The study found that adding TENS (搜索) to standard physical therapy resulted in a clinically meaningful reduction in movement-evoked pain. At the 60-day primary endpoint, participants receiving TENS therapy experienced a 1.2-point greater reduction in movement-evoked pain on a 10-point scale compared to those receiving physical therapy alone (95% CI, −1.6 to −0.7; d = 0.46).
"It is one of the few treatments that specifically targets movement-evoked pain and fatigue, which are major barriers to participation in daily activities," says Sluka, UI professor of physical therapy and rehabilitation science.
The treatment demonstrated a clear dose-response relationship, with participants who used TENS (搜索) daily for 60 days achieving the best outcomes. Notably, 72% of patients in the TENS group reported improvement compared to 51% in the physical therapy-only group (P = .001).
Breakthrough in Fatigue Management
Perhaps most significantly, the study showed that TENS (搜索) therapy also reduced fatigue—a primary symptom of fibromyalgia (搜索) that has few effective treatments. "We were excited to see that patients also had less fatigue," Sluka noted. "Right now, there are no good treatments for fatigue. So, the fact that we had anything that touched the fatigue was pretty powerful."
The research addressed a fundamental paradox in fibromyalgia (搜索) care: while exercise is considered the best medicine for the condition, movement itself causes agonizing pain and crushing fatigue for patients.
Real-World Effectiveness and Safety
Unlike many laboratory-based studies, FM-TIPS was conducted in everyday clinical settings with participants of diverse ages, education levels, and socioeconomic backgrounds. Nearly 50% of participants were from rural areas, demonstrating the treatment's broad applicability.
The study protocol required participants in the TENS (搜索) group to use the device for two hours daily for six months, with electrodes placed on the upper and lower back delivering mixed frequency signals at the strongest tolerable intensity.
Safety data showed no serious adverse events, with only 30% of participants experiencing minor adverse events over the six-month study period. This compares favorably to FDA-approved fibromyalgia (搜索) medications, which often carry significant side effect profiles.
Sustained Long-Term Benefits
Unlike pain medications that can lose effectiveness over time due to tolerance, TENS (搜索) maintained its therapeutic benefits throughout the study period. At six months, 81% of participants found TENS helpful, and 55% continued using it daily.
Dana Dailey, PT, PhD, the study's first author and UI assistant research scientist, emphasized that TENS (搜索) works best as part of a comprehensive treatment approach. "Using TENS on its own will not give the same benefits," Dailey explains. "However, the study shows that TENS provides an added benefit on top of any relief from other treatments."
Clinical Implementation and Accessibility
The research demonstrates that TENS (搜索) offers several advantages for fibromyalgia (搜索) management: it's safe, inexpensive, readily available, and can be used as a self-management tool. The treatment showed effectiveness even when participants were simultaneously using pain medications and receiving physical therapy.
"Often, when you move a randomized, controlled clinical trial into a real-world setting, it doesn't work because there are too many confounding factors. But this intervention still works," Sluka observed. "Not only did the treatment reduce movement pain and fatigue during the testing period, but patients continued to use it at six months."
The study's design allowed the physical therapy-only group to receive TENS (搜索) devices after the 60-day primary endpoint, and these participants showed the same improvements as the original TENS group, further validating the treatment's effectiveness.
Addressing an Unmet Medical Need
Fibromyalgia (搜索) affects approximately 4% to 7% of the population and significantly impacts physical function, cognitive abilities, and sleep quality. The condition's hallmark features include chronic widespread pain and whole-body fatigue that interferes with daily activities and contributes to concentration difficulties.
