The drug, codenamed TERN-701, showed superior molecular response rates compared to existing therapies in patients who had previously failed multiple treatments.
Strong Efficacy Results in Treatment-Experienced Patients
In the CARDINAL study, TERN-701 helped 74% of 38 evaluable patients achieve a major molecular response by 24 weeks. This represents a 1,000-fold reduction in the percentage of white blood cells containing the aberrant BCR-ABL (搜索) gene that drives CML (搜索). Among patients who weren't classified as being in major molecular response at the study's start, 64% achieved it after treatment, while all patients who were already at that level maintained their status.
The results appear particularly impressive when compared to Novartis' Scemblix, which helped 25% of trial volunteers achieve a major molecular response at 24 weeks in a similar study population. While cross-trial comparisons can be misleading and the two drugs haven't been tested head-to-head, the difference is substantial.
"What it's telling us is that this is a better drug," said Terns CEO Amy Burroughs in an interview. "This is going to raise the bar in efficacy."
Favorable Safety Profile
The updated safety data presented at ASH showed that 32% of the 63 patients who entered the trial experienced a Grade 3 adverse event, with only one patient dropping out due to a severe side effect. Notably, no signs of high blood pressure or pancreatitis have occurred with TERN-701, side effects that are associated with Scemblix and another CML (搜索) drug, Iclusig. Iclusig also carries a black box warning for blood clotting events that have included heart attacks and strokes.
Novel Mechanism of Action
TERN-701 belongs to a class of drugs that work by blocking the signaling molecule responsible for producing diseased white blood cells in CML (搜索). While early drugs like Gleevec worked by blocking the active part of the molecule, both Scemblix and TERN-701 use a different approach, grabbing onto a site that changes the shape of the molecule. This mechanism reduces the number of "off-target" interactions that can trigger severe side effects, according to Burroughs.
Study Population and Design
The CARDINAL study is primarily evaluating the safety of TERN-701 and determining the optimal dose for further testing. Nearly all study participants joined the trial because their previous treatments had stopped working or caused intolerable side effects. The 28 evaluable patients had already been treated with a median of three prior drugs.
The study measured molecular response rates, which are frequently used as endpoints in CML (搜索) trials because they predict longer-term benefits such as reduced risk of disease progression. This makes the strong response rates particularly meaningful for assessing the drug's potential clinical impact.
Market Implications
The promising results have generated significant investor interest, with Terns shares more than tripling since the company first announced preliminary data in early November. Analysts have suggested that TERN-701 "could potentially upend Scemblix dominance" in the CML (搜索) market, where Novartis' drug is expected to generate more than $4 billion in peak yearly sales.
