Tildrakizumab Demonstrates Sustained Safety Profile Over Four Years in Psoriatic Arthritis Extension Study
核心洞察
Phase 2b open-label extension study confirms tildrakizumab maintains favorable safety profile over 208 weeks in 281 psoriatic arthritis (搜索) patients.
Only 6.0% of patients discontinued treatment due to adverse events, with most experiencing mild to moderate side effects like upper respiratory infections.
IL-23 (搜索) inhibitors show lower serious infection risk compared to other biologics, positioning them as preferred choice for high-risk patients.
Long-term safety data for the IL-23 (搜索) inhibitor tildrakizumab in psoriatic arthritis (搜索) patients demonstrates sustained tolerability over more than four years of treatment, according to findings presented at the American College of Rheumatology Convergence. The phase 2b open-label extension study, encompassing 281 participants followed for 208 weeks, reported no new safety signals and adverse events consistent with prior findings.
Christopher Ritchlin, MD, MPH, professor and chief of the Division of Allergy, Immunology and Rheumatology at the University of Rochester Medical Center, analyzed the sustained safety and durability demonstrated by the findings. According to Ritchlin, the safety profile represents a major advantage of IL-23 (搜索) inhibition in psoriatic arthritis (搜索), as these agents are not associated with the same significant risk of infections often seen with other biologic treatments.
Safety Profile Advantages
The extended safety analysis revealed that while 79.7% of participants experienced an adverse event, most were mild or moderate in severity. The most common adverse events included upper respiratory tract infection and nasopharyngitis. Notably, only 6.0% of patients discontinued treatment due to adverse events, demonstrating good tolerability over the extended treatment period.
Serious adverse events were infrequent, occurring in 14.2% of patients, with very few (0.7%) considered treatment-related. The study found that rare cardiac disorders and malignancies were not linked to the study drug. The risk of serious infections is lower with IL-23 (搜索) inhibitors compared to other biologics, and there is no comparable risk of Candida infection that can be observed with IL-17 inhibitors.
Clinical Positioning and Future Applications
The sustained long-term safety data strengthens the positioning of IL-23 (搜索) inhibitors as a preferred choice, especially for patients with comorbidities or a history of infection. The safety advantage of the anti-IL-23 class makes these agents particularly important for patients who are at higher risk of developing infections, such as those with diabetes (搜索) or those who have experienced infectious complications with other biologic therapies.
Combination Therapy Potential
IL-23 (搜索) inhibitors demonstrate versatility for combination therapy, notably with anti-tumor necrosis factor (TNF (搜索)) agents. This area of research is rapidly expanding, with studies already completed, such as the AFFINITY-1 trial combining guselkumab with golimumab. An ongoing study is evaluating the combination of another anti-IL-23, deucravacitinib, with an anti-TNF agent for psoriatic arthritis (搜索), suggesting that combining tildrakizumab with other biologics is a likely future therapeutic strategy.
Efficacy Foundation
The long-term safety data builds on tildrakizumab's established efficacy profile from two successful Phase 3 trials (INSPIRE-1 and INSPIRE-2) that met their primary endpoint of ACR20 response at 24 weeks. The sustained long-term safety and durability profile demonstrated by this data offers compelling evidence supporting continued development for psoriatic arthritis (搜索).
Future Research Directions
To fully understand tildrakizumab's long-term role in psoriatic arthritis (搜索), additional endpoints in clinical trials are crucial. Radiographic progression data is important not only to rheumatologists but also to dermatologists, as evidenced by recent studies like the APEX trial with guselkumab. Quality of life scores are essential, and given that IL-23 (搜索) is hypothesized to be a major driver of enthesitis (搜索), more complete enthesitis data is needed, ideally within phase 3 trials. Pain reduction represents another vital area where IL-23 inhibitors have shown promise, and more specific data on pain reduction with tildrakizumab in psoriatic arthritis patients would significantly enhance its clinical profile.
