Tocilizumab Shows Promise as First Potential Treatment for Deadly Hantavirus Infections

A rheumatoid arthritis (搜索) drug has shown promising results as a potential treatment for hantavirus (搜索) infections, offering hope for addressing a deadly disease that currently has no approved therapies. Researchers in Argentina reported that tocilizumab helped four of five patients survive hantavirus pulmonary syndrome (搜索), according to findings published in The Lancet Infectious Diseases.
The study tracked the first patients to receive tocilizumab in an ongoing "compassionate use" trial at an Argentinian hospital. In contrast, five patients who were deemed eligible for the drug but didn't receive it died after receiving only standard supportive care. Two patients worsened too quickly for treatment, while the hospital lacked sufficient supply for the others.
Targeting Inflammatory Response
Tocilizumab works by suppressing IL-6, a molecule that triggers damaging inflammation in autoimmune diseases. Researchers believe IL-6 also plays a key role in the inflammatory reaction to hantavirus (搜索) infection, which can rapidly cause lungs to fill with fluid and fail.
"The research team cautioned that the five patients who didn't receive the drug were sicker and older than those who did," noted Dr. Fernando Tortosa of the National University of Río Negro (搜索) in Patagonia, Argentina, the study's lead author. "Still, they said tocilizumab warrants further investigation."
Urgent Need for Treatment Options
The research comes amid renewed attention to hantavirus (搜索) following a deadly cruise ship outbreak that killed three of 13 infected passengers. The outbreak involved the Andes virus (搜索), the only hantavirus strain thought capable of human-to-human transmission in some cases.
Hantavirus (搜索) infections, while rare, carry devastating mortality rates. In Chile, authorities have confirmed 15 deaths among 42 cases this year, while Argentina has reported 32 deaths among 102 cases since June 2025. In the United States, 35% of hantavirus cases since tracking began in 1993 have resulted in death, according to the CDC.
In Canada, approximately five to 10 cases occur annually, with about 40% proving fatal, according to Health Canada data. The majority of Canadian cases occur in Saskatchewan and Alberta.
Multiple Research Approaches
Beyond tocilizumab, several teams are pursuing antibody-based treatments. Dr. María Inés Barría's team at Universidad San Sebastián (搜索) in Chile, working with researchers from the U.S. National Institutes of Health (搜索)'s Rocky Mountain Laboratories and Germany's Robert Koch Institute, is developing cloned antibodies from hantavirus (搜索) survivors.
The team published research in 2018 showing their approach worked in animals, but funding for human trials was diverted during the coronavirus pandemic. "We are truly at the forefront, at a very important stage of moving to the next phase," Barría said.
Additional antibody research is underway at the Albert Einstein College of Medicine and the Vanderbilt Center for Antibody Therapeutics (搜索).
Vaccine Development Challenges
Vaccine development faces significant obstacles despite ongoing efforts. A team led by Jay Hooper of the U.S. Army Medical Research Institute of Infectious Diseases (搜索) has developed a vaccine that successfully generated antibodies against the Andes virus (搜索) in early-stage human trials, according to 2020 research.
However, Dr. Paul Bollyky of Stanford Medical Center highlighted the fundamental challenges facing hantavirus (搜索) research. "Labs typically don't have the necessary machinery they need to test and validate vaccines and treatments for rare infections," he explained. "Because hantavirus outbreaks are so sporadic and unpredictable, that virus is much harder to study compared with a common germ that regularly circulates."
Research at VIDO
At the University of Saskatchewan's Vaccine and Infectious Disease Organization (搜索) (VIDO), Dr. Bryce Warner is exploring why deer mice, the primary hantavirus (搜索) carriers, remain healthy while humans develop fatal illness.
"When deer mice are infected with the virus, they don't get sick," Warner explained. "They don't show any clinical signs of illness. And so they carry it for life and they don't seem to be harmed at all, which is obviously different from when humans become infected."
The VIDO research aims to identify biomarkers that could be targeted with treatments by comparing immune responses between deer mice and humans. Warner noted that his lab is working on vaccine approaches currently in early clinical or pre-clinical studies.
Renewed Hope for Investment
Researchers hope the cruise ship outbreak will catalyze increased investment in hantavirus (搜索) research. "I hope this situation will help us continue our research and strengthen the collaboration between healthcare workers, the community, and the necessary resources," Tortosa said.
The challenge extends beyond scientific obstacles to economic realities. Because hantavirus (搜索) infections are unpredictable and affect small numbers of people, there may not be a large or steady market for vaccines or treatments, making sustained investment difficult to secure.
Despite these challenges, researchers remain committed to developing solutions for what Barría calls "a public health problem" due to its extreme lethality. As climate change is expected to increase contact between people and rodents, the urgency for effective hantavirus (搜索) treatments continues to grow.
