Targeting Inflammation in Depression
The four-week proof-of-concept trial enrolled 30 participants with moderate-to-severe depression (搜索) who had not responded well to standard antidepressants and showed signs of low-grade inflammation in two separate blood tests taken two weeks apart. Participants were randomly assigned to receive either tocilizumab (14 people) or a placebo (16 people).
Current drug treatments for depression (搜索) exclusively target brain chemicals such as serotonin, norepinephrine, and dopamine. However, approximately one in three people with depression do not improve with these conventional treatments. Recent research indicates that about one-in-three people with depression have signs of inflammation in their blood, suggesting that for some patients, symptoms may be linked to an overactive immune system.
Promising Clinical Outcomes
While the small study size limited statistical significance, participants who received tocilizumab demonstrated greater improvements over time across multiple measures compared to those given placebo, including overall depression (搜索) severity, fatigue, state anxiety, and quality of life. The most substantial effects were observed at the final follow-up on day 28.
Most notably, the tocilizumab group achieved significantly higher remission rates, with 54% of patients reaching clinical depression (搜索) remission compared to just 31% in the placebo group. This translates to a Number Needed to Treat (NNT) of 5, meaning only five patients need treatment for one additional person to achieve full remission. This markedly outperforms first-line SSRI (搜索) antidepressants, which carry an NNT of approximately 7.

Dr Éimear Foley, Senior Research Associate in Immunopsychiatry at Bristol's MRC IEU and the study's lead author, noted the broader implications: "Depression (搜索) is estimated to affect around 10-20% of people worldwide during their lifetime, yet for many patients current treatments do not work well enough. Our study moves us closer to more tailored depression care, where treatments are chosen to better fit a person's biology."
The research revealed that baseline high-sensitivity C-reactive protein (hs-CRP) levels, rather than IL-6 levels, better tracked depression (搜索) improvement, suggesting hs-CRP may serve as a more reliable predictor of immunotherapy response in depression than drug-specific biomarkers.
Safety and Tolerability
Tocilizumab was well tolerated throughout the trial, with no serious adverse events or patient withdrawals reported. This safety profile aligns with the drug's established use in treating autoimmune conditions.
The next phase will involve conducting a large-scale phase III randomized controlled trial to provide definitive evidence that could enable doctors to prescribe immunotherapy for depression (搜索). The current study provides crucial data on treatment-sensitive outcomes, effect sizes, and patient selection methods for testing systemic IL-6 inhibition in patients with difficult-to-treat depression.
