Traws Pharma's Ratutrelvir Shows Promise as Ritonavir-Free COVID-19 Treatment in Phase 2 Trial

Traws Pharma (搜索) announced positive interim results from its ongoing Phase 2 clinical trial comparing ratutrelvir, an investigational oral ritonavir-free protease inhibitor, to PAXLOVID in patients with mild-to-moderate COVID-19 (搜索). The randomized, open-label study demonstrated that ratutrelvir achieved comparable symptom resolution while showing improved tolerability and no viral rebound events.
Clinical Trial Design and Patient Population
The active-controlled comparator trial enrolled 37 patients in the interim analysis, with 25 receiving ratutrelvir and 12 receiving PAXLOVID. The study represents more than 50% of the planned 90-patient population. Patients in the ratutrelvir arm received 600 mg orally once daily for 10 days, while the comparator group received standard PAXLOVID dosing of nirmatrelvir 300 mg plus ritonavir 100 mg twice daily for 5 days.
A notable aspect of the trial included a separate treatment arm for patients ineligible for ritonavir-boosted regimens due to contraindications or clinically significant drug-drug interactions. Six patients (24% of the ratutrelvir cohort) fell into this category, representing a significant population with limited effective treatment options.
Efficacy Outcomes
Ratutrelvir-treated patients demonstrated time-to-sustained symptom alleviation and resolution that was numerically comparable to PAXLOVID-treated patients, as measured using the FLU-PRO Plus/COVID-19 (搜索) Symptoms Diary. Sustained alleviation was defined as self-reported alleviation of all COVID-19 symptoms for four consecutive days.
"From a clinical perspective, these interim data suggest that ratutrelvir may provide a meaningful benefit across a broader range of patients, including those who are unable to receive ritonavir-boosted therapy," commented Robert Redfield, MD, Chief Medical Officer at Traws Pharma (搜索).
Importantly, no COVID-19 (搜索) symptom or virologic rebound events were observed in ratutrelvir-treated patients, while one rebound event occurred in the PAXLOVID comparator arm (1 of 12 patients; 8.3%) shortly after completion of the standard 5-day dosing regimen.
Safety and Tolerability Profile
Ratutrelvir demonstrated superior tolerability compared to PAXLOVID, with fewer reported adverse events. The most commonly reported adverse event among ratutrelvir-treated patients was mild dyspepsia in 2 patients (7.6%). Notably, no dysgeusia (distorted sense of taste) or ritonavir-associated adverse effects were reported, and no treatment discontinuations due to adverse events occurred.
In contrast, adverse events commonly associated with PAXLOVID, including dysgeusia, dizziness, and dyspepsia, were reported in 4 patients (30%) in the comparator arm, consistent with prior clinical trial and real-world experience.
Mechanism and Pharmacokinetic Profile
Ratutrelvir is an investigational oral, small molecule Mpro (3CL protease (搜索)) inhibitor designed as a broadly acting treatment for SARS-CoV-2 (搜索)/COVID-19 (搜索) without requiring ritonavir co-administration. Phase 1 data showed that ratutrelvir's pharmacokinetic profile maintained target blood plasma levels approximately 13 times above the EC50 using the target Phase 2 dosing regimen of 600 mg/day for ten days.
This enhanced pharmacokinetic profile may reduce the likelihood of clinical rebound and consequently reduce the risk for Long COVID (搜索), according to the company's analysis.
Implications for Long COVID Prevention
"The combination of early and sustained symptom improvement, extended dosing duration, absence of viral rebound observed to date, and favorable tolerability supports the strategic hypothesis that ratutrelvir may have utility in reducing post-acute sequelae of SARS-CoV-2 (搜索) infection (Long COVID (搜索))," commented Dr. Redfield.
The extended 10-day dosing regimen and ritonavir-free formulation may enable earlier and potentially more complete viral clearance without the limitations associated with ritonavir boosting, offering a differentiated approach to both acute COVID-19 (搜索) treatment and prevention of longer-term complications.
