The Triceps trial, led by researchers at the University of Sheffield and Sheffield Teaching Hospitals NHS Foundation Trust, is evaluating transcutaneous vagus nerve stimulation (搜索) (TVNS (搜索)) as a novel approach to improving hand and arm function in stroke (搜索) survivors. The £2 million study, launched two years ago, is now running across 19 NHS centers nationally and needs just 65 more patients to complete its expanded recruitment target of 270 participants.
Revolutionary Non-Invasive Approach
The treatment represents a significant advancement from previous vagus nerve (搜索) stimulation research, which required surgical implantation of electrical stimulators in the chest. TVNS (搜索) uses a wearable device that connects to a wired earpiece to deliver electrical pulses through the skin to stimulate the vagus nerve, a key pathway that can influence damaged brain regions involved in motor control.

However, in these trials the electrical stimulator was surgically implanted in the chest," according to the trial documentation. Addressing Significant Unmet Need
The trial targets a substantial patient population, as approximately one-third of stroke (搜索) survivors experience permanent hand and arm weakness (搜索) following their stroke, severely limiting their ability to perform everyday activities. This disability significantly impacts independence, quality of life, and overall recovery outcomes.
Professor Arshad Majid, Professor of Cerebrovascular Neurology at the University of Sheffield and Consultant Neurologist at Sheffield Teaching Hospitals NHS Foundation Trust, emphasized the clinical significance: "Stroke (搜索) is the leading cause of severe disability and can have a huge, long-lasting effect on someone's life. This study offers real hope for people living with arm weakness (搜索) after stroke, a highly disabling problem that affects independence, quality of life, and recovery for many stroke survivors."
Trial Design and Patient Population
The double-blind, placebo-controlled study is open to adults over 18 years old who experience arm and hand weakness (搜索) from strokes that occurred between six months and ten years ago and are not currently undergoing rehabilitation therapy. Neither patients nor researchers know whether participants receive active TVNS (搜索) treatment or placebo during the trial period.
Participants wear the device for one hour, five times per week during home stroke (搜索) rehabilitation with support from community therapists. Some participants also use the device during daily activities over a 12-week period.
The trial originally planned to recruit 243 patients but expanded to 270 following an interim independent analysis reviewed by an external oversight committee, indicating promising early signals that warranted studying a larger patient population.
Path to Widespread Implementation
With 205 patients already recruited and only 65 more needed to reach the target, the trial is approaching completion of its recruitment phase. Professor Majid noted the urgency of finishing enrollment: "We're really excited to be close to the final stages of this part of the study, as the sooner we finish recruiting; the sooner we can seek to make this promising new treatment more widely available to stroke (搜索) survivors throughout the country."
The trial's success could lead to the rollout of TVNS (搜索) treatment "at cost and at scale" across the healthcare system, potentially providing a cost-effective solution for the substantial population of stroke (搜索) survivors living with persistent arm and hand weakness (搜索).
The study is supported by the NIHR Sheffield Biomedical Research Centre (搜索), NIHR Sheffield Clinical Research Facility, and the Sheffield Stroke (搜索) and Aphasia Interest Group, which has been involved in trial design and will continue supporting the research throughout its duration.
