Cuban, a longtime Trump critic who has praised this initiative, explained that the dramatic price reductions are possible because the program eliminates pharmacy benefit managers (PBMs) as intermediaries.
"Anything that impacts the middleman, I'm a fan of," Cuban told Business Insider Thursday. "The big brand pharmaceutical manufacturers recognize they make more money with net pricing. That's why Lilly and Novo do their direct thing, and it's half the price, or less than half the price, of what a corporate sponsor is paying for their GLP-1 (搜索), even after rebates."
PBMs traditionally negotiate with drug manufacturers to include medications in their formularies and secure patient coverage. This opaque process creates wildly different pricing depending on insurance coverage, making it difficult for consumers to predict prescription costs until reaching the pharmacy counter.
Manufacturing Investments Enable Lower Pricing

Cuban's Parallel Cost-Cutting Initiative
Coinciding with Trump's announcement, Cuban's Cost Plus Drugs (搜索) revealed its own major price reduction for Stelara, an autoimmune injectable medication. The pharmacy will offer the generic version Starjemza (搜索) for approximately $1,400 annually, down from Stelara's roughly $80,000 yearly cost.
This represents Cuban's latest effort to eliminate PBMs from healthcare pricing. His Cost Plus Drugs (搜索) operates on "net pricing"—providing transparent, fully negotiated prices for all customers rather than using inflated list prices subject to middleman negotiations.
Industry Transformation Toward Direct Sales
Cuban describes both initiatives as part of a "farm-to-table" transformation in prescription drug pricing, featuring clear price tags and direct-to-consumer sales models.
"TrumpRx fits that mold," Cuban said. "It's not that the brands don't want to sell to us—it's that they're used to selling through middlemen. Now, with direct-to-consumer happening more and more, or farm-to-table, I think that's going to start to change."
The billionaire entrepreneur noted that pharmaceutical companies are increasingly willing to engage in direct conversations about pricing. "We're having a lot of really good conversations directly with pharmaceutical brands," Cuban said.
Cuban's ultimate goal involves making every brand-name medication available with transparent pricing, eliminating the uncertainty that currently characterizes prescription drug costs for American consumers.
