UC Cancer Center Launches Phase 3 Trial Testing High-Dose Radiation for Locally Advanced Pancreatic Cancer

Addressing an Unmet Medical Need
Patients with locally advanced pancreatic cancer (搜索) face significant challenges, as tumor location often disqualifies them from surgical intervention. According to Dr. Jordan Kharofa, site principal investigator at the Cancer Center and professor and vice chair of research in the UC College of Medicine's Department of Radiation Oncology, current standard of care includes chemotherapy (搜索) alone or in combination with low-dose radiation, but these treatments typically only slow tumor growth without significantly impacting patient survival.
"There's really been no great standard to treat these patients," said Kharofa, who also serves as senior advisor and chair of the Protocol Review and Monitoring System at the Cancer Center. For patients in this cohort, three-year survival rates are approximately 15%.
Novel Radiation Technique with Precision Targeting
The trial centers on an innovative radiation approach that addresses the fundamental limitation of current treatments. Traditional radiation therapy (搜索) for pancreatic cancer (搜索) patients is restricted to low doses to protect healthy surrounding tissues and organs, particularly the bowel. The new technique involves placing markers on tumors endoscopically, enabling precise targeting during treatment delivery.
"We'll have the patient hold their breath, typically, and while they're holding their breath, we can see on the X-rays while they're on the machine that the markers are in exactly the right spot," Kharofa explained. "So with all that confidence in localization, we can increase the radiation dose. We can carve various deep shapes around the bowel, and with more confidence deliver these ablative, or cancer-killing, radiation doses."
Trial Design and Enrollment
The randomized Phase 3 trial will enroll approximately 350 patients across the country. Participants will be randomized to receive either the new high-dose radiation treatment guided by tumor markers or current standard treatments consisting of chemotherapy (搜索) with or without standard-dose radiation.
"So if patients are in the randomized arm getting the newer ablative dose, the dose is high enough that we hope it can meaningfully impact the tumor compared to what we've been able to do in the past," Kharofa said.
Promising Early Data
The trial builds on encouraging preliminary results from Memorial Sloan Kettering Cancer Center, where this marker-guided radiation technique was pioneered. Early data showed survival rates for patients with locally advanced unresectable tumors comparable to those achieved in patients who underwent surgical tumor removal.
Researchers will primarily evaluate three-year survival rates as the primary endpoint to measure the trial's success. "There's a lot of excitement about this, and hopefully it will become a standard option if it's successful," Kharofa said.
