Researchers at the University of California, Davis, have demonstrated that mesenchymal stromal cell (MSC) therapy combined with antiviral drugs (搜索) significantly enhances immune recovery in cats with feline infectious peritonitis (搜索) (FIP (搜索)), a severe coronavirus infection (搜索). The findings, published in STEM CELLS Translational Medicine, offer promising insights for treating long-term immune dysfunction following coronavirus infections in both veterinary and human medicine.
FIP as a Model for Coronavirus-Related Immune Dysfunction
Feline infectious peritonitis (搜索) is a historically fatal disease affecting approximately 2% of cats, most commonly young cats in shelters or catteries. The disease is caused by a coronavirus that typically infects intestinal cells but can mutate to infect immune cells (搜索) and spread throughout the body. FIP (搜索) manifests in two forms: wet FIP, characterized by fluid accumulation in the chest or belly, and dry FIP, where inflammation affects organs without fluid buildup.
"Our findings support the idea that FIP (搜索) in cats is a useful model for studying long-term immune problems after a viral infection — something that also happens in some people after illnesses like COVID-19 (搜索)," said corresponding author Amir Kol, associate professor in the Department of Pathology, Microbiology and Immunology at the UC Davis (搜索) School of Veterinary Medicine.
Both forms of FIP (搜索) cause fever, widespread inflammation, problems in multiple organs, and a significant drop in important immune cells (搜索). The condition shares several critical features with severe COVID-19 (搜索) infections in humans, including widespread inflammation, T cell exhaustion, and chronic immune dysfunction.
Clinical Trial Design and Treatment Protocol
The clinical trial divided cats with FIP (搜索) into two groups: one receiving antiviral drugs (搜索) combined with MSC infusions, and another receiving antiviral drugs with placebo infusions. All cats in the study had enlarged lymph nodes, indicating ongoing immune system activation even after apparent recovery from the acute infection.
"As expected, all cats survived because the antiviral treatment is effective in getting rid of the virus," Kol explained. "But we definitely saw evidence of immune recovery in those cats that were treated with MSC compared to the ones that weren't."
The antiviral agent used in the study was GS-441524, which UC Davis (搜索) researchers first reported as successful in curing FIP (搜索) in 2018. However, the drug only became available through U.S. veterinary compounding pharmacies within the last year due to patent restrictions.
Immune System Recovery and Long-Term Effects
The combination therapy produced measurable improvements in immune function. Cats receiving MSC therapy (搜索) showed fewer overly active T and B cells (搜索), which can cause immune overdrive, and demonstrated increased numbers of regulatory T cells (搜索) that help calm the immune system. Additional testing confirmed that the therapy helped exhausted immune cells (搜索) recover and potentially form long-term memory cells for future protection.
The persistence of enlarged lymph nodes in some cats after FIP (搜索) recovery suggests a "long-FIP"-like syndrome, similar to long COVID (搜索) in humans. "Even if you treat the infection with antivirals, your immune system, oftentimes, still won't go back to normal or where it was before infection," Kol noted. "This might have long-lasting effects on your health and this study shows that MSC may help."
Safety Profile and Clinical Implications
The MSC therapy (搜索) demonstrated an excellent safety profile with no serious side effects reported. The treatment not only reduced short-term inflammation but appeared to help restore lasting immune system balance, addressing the chronic immune dysfunction that can persist after severe coronavirus infections.
"It's important to understand that many pets suffer from diseases that are similar to human diseases," Kol emphasized. "Veterinary medicine is a wonderful platform to conduct translational science that helps both pets and humans."
