UK Launches Expanded Type 1 Diabetes Screening Program as Genetic Risk Tools Show Promise for Early Detection

The UK has launched an expanded childhood screening program for type 1 diabetes (搜索) while researchers demonstrate that genetic risk scoring can effectively identify adults at highest risk for the autoimmune condition, potentially transforming early detection and treatment approaches as new immunotherapies become available.
ELSA Study Demonstrates Screening Effectiveness
The Early Screening for Autoimmune Diabetes (ELSA) study, launched in 2022 as the first UK study of its kind, tested blood samples from 17,931 children aged 3-13 for autoantibodies (搜索) that can appear years before type 1 diabetes (搜索) symptoms develop. Among the 17,283 children screened, researchers identified 75 children with one autoantibody indicating increased future risk, 160 children with two or more autoantibodies representing early-stage type 1 diabetes, and seven children with undiagnosed type 1 diabetes requiring immediate insulin therapy.
"We are extremely grateful to all the families who have participated in the study and generously given their time to help understand how a UK-wide screening programme could be developed," said lead researcher Parth Narendran, Professor of Diabetes Medicine at the University of Birmingham. "Together with Diabetes UK (搜索), Breakthrough T1D (搜索) and the National Institute for Health and Care Research, we are working towards a future where type 1 diabetes (搜索) can be detected in a timely manner, and families appropriately supported and treated with medicines to delay the need for insulin."
Children without autoantibodies (搜索) are unlikely to develop type 1 diabetes (搜索), while those with one autoantibody have a 15% chance of developing the condition within 10 years. Having two or more autoantibodies indicates the immune system has already started attacking insulin-producing cells in the pancreas, making insulin therapy almost certain.
Genetic Risk Score Advances Adult Screening
Parallel research published in The Lancet Diabetes and Endocrinology demonstrates that a genetic risk score can identify adults at highest risk for developing type 1 diabetes (搜索). The international team applied the genetic risk score to adults in the Exeter 10,000 cohort and showed that people scoring in the top 10% were at eight times higher risk of being in the early stages of type 1 diabetes.
"Now that new drugs are available that can delay the onset of diabetes, we urgently need to be able identify who is at high risk of developing the condition, particularly as the drug is only approved before symptoms occur," said Professor Richard Oram at the University of Exeter, who developed the genetic risk score. "Our research shows us that we could use this score in large population studies such as Our Future Health to identify who is at the highest risk and offer them autoantibody testing, as an efficient way of identifying adults with early-stage type 1 diabetes (搜索) who may be eligible for treatment."
The genetic risk score feeds into an online clinical calculator available to clinicians worldwide, which helps identify children at high risk by incorporating routine clinical information including age, family history, blood test results, and genetic risk scores.
Teplizumab Offers Treatment Hope
The screening initiatives gain urgency as teplizumab, the first immunotherapy for type 1 diabetes (搜索), becomes available. Licensed by the UK's Medicines and Healthcare products Regulatory Agency in August 2025, the drug can delay the need for insulin by around three years in people with early-stage type 1 diabetes. The treatment is currently under review by the National Institute for Health and Care Excellence to determine NHS availability.
Crucially, teplizumab only works when administered before symptoms develop, making early identification essential. Some ELSA study families were offered the treatment, with the first patient treated at Birmingham Children's Hospital.
Amy Norman, whose 11-year-old daughter Imogen became the second child in the UK to access teplizumab through ELSA, described the impact: "Being part of the ELSA study has helped us as a family to prepare for the future in a way we never expected. Knowing what's coming – rather than being taken by surprise – has made an enormous difference to our confidence and peace of mind."
ELSA 2 Expansion
Building on ELSA's success with over 37,000 families enrolled, ELSA 2 launches today to expand screening to all UK children aged 2-17 years, focusing on younger children (2-3 years) and older teenagers (14-17 years). The research team aims to recruit 30,000 additional children across these age groups.
ELSA 2 will establish new NHS Early-Stage Type 1 Diabetes (搜索) Clinics, providing families with clinical and psychological support and creating clear pathways from screening to diagnosis, monitoring and treatment.
Clinical Impact and Future Implications
The screening approach addresses a critical healthcare challenge, as type 1 diabetes (搜索) affects up to 400,000 people in the UK and often presents as a medical emergency. Families of children identified with early-stage disease receive tailored education and ongoing support to prepare for eventual symptom onset, reducing emergency treatment needs.
