Ultrasensitive Blood Test HPV-DeepSeek Predicts Recurrence and Survival in HPV-Associated Head and Neck Cancer

Two new studies are advancing the understanding of how blood-based tests measuring circulating tumor HPV DNA (搜索) could personalize treatment for patients with HPV-associated head and neck cancer (搜索), offering the prospect of moving beyond generalized risk estimates toward molecularly guided care.
An ultrasensitive blood test called HPV-DeepSeek (搜索), developed by investigators at Mass General Brigham Cancer Institute (搜索), has demonstrated the ability to predict recurrence and survival after surgery, according to findings published in Science Translational Medicine. Separately, researchers at The Ohio State University Comprehensive Cancer Center (搜索) – Arthur G. James Cancer Hospital and Richard J. Solove Research Institute (OSUCCC – James) have published work in JAMA Otolaryngology – Head and Neck Surgery examining how tumor biology and patient factors influence ctDNA levels before and after surgery.
HPV-DeepSeek (搜索) demonstrates high sensitivity and prognostic power
The Clear-HPVca study followed 103 people with newly diagnosed, untreated HPV-associated head and neck cancer (搜索) scheduled for surgery at Mass General Brigham Cancer Institute (搜索) between August 2020 and March 2024. Researchers collected and analyzed 560 blood samples before surgery, after surgery, and during surveillance, following patients for more than two years. Of those patients, 73% received additional treatments after surgery, including radiation or chemoradiation.
At diagnosis, HPV-DeepSeek (搜索) detected circulating tumor HPV DNA (搜索) in 98.1% of patients. In the critical post-surgical testing window, 23% of patients had detectable circulating tumor HPV DNA. Compared to an existing HPV blood test, HPV-DeepSeek was substantially more sensitive.
The prognostic implications were striking. Patients with positive HPV-DeepSeek (搜索) results after surgery had significantly worse outcomes: only 60% remained disease-free at two years, compared with 100% of patients with negative tests. Overall survival similarly diverged — 73% of patients with detectable tumor HPV DNA were still alive at the end of the trial, compared with 98% of those with negative HPV-DeepSeek results after surgery.
"After surgery for HPV-associated head and neck cancer (搜索), we currently rely on very general clinical risk factors to determine which patients need more treatment and which patients do not," said senior author Daniel Faden, MD, Director of the Head and Neck Cancer Genomics and Liquid Biopsy Program at Mass General Brigham Cancer Institute (搜索). "That means some patients receive more treatment than they actually need, resulting in more side effects of treatment, while others receive less treatment than they should — and later have their cancer come back."
Early detection of recurrence
HPV-DeepSeek (搜索) also demonstrated the ability to detect cancer recurrence earlier than traditional methods and existing HPV DNA-based blood tests — approximately seven months before clinical detection, nearly double the lead time of existing HPV DNA blood tests, and extending up to 17.5 months.
"What excites me most about this approach is the possibility of moving beyond generalized risk estimates and toward truly personalized care," said Faden. "If we can accurately identify residual disease at the molecular level, we may eventually be able to make treatment decisions based on the biology of an individual patient's cancer rather than broad clinical categories."
The study authors noted limitations: the research was observational, meaning investigators only collected data without using it to make treatment decisions. The study was also conducted within a single healthcare system, with a small number of patients whose cancer recurred or who died. The team is now planning larger, multi-site clinical trials to guide post-surgical treatment selection.
Ohio State study emphasizes contextual interpretation
The complementary study from OSUCCC – James followed 104 adult patients treated for HPV-associated throat cancer between September 2021 and April 2025. Twenty patients were female and 84 were male, and most had early-stage tumors located in the tonsils. All patients underwent surgery, with additional therapy — radiation and/or chemotherapy — recommended based on pathologic risk factors. Circulating tumor HPV DNA (搜索) was tested before surgery in all patients and after surgery, prior to radiation, in 74 patients.
"We found that the pretreatment ctDNA levels are influenced by tumor biology and kidney function," said Catherine Haring, MD, an otolaryngologist specializing in head and neck cancers and assistant professor in the Department of Otolaryngology – Head and Neck Surgery at The Ohio State University College of Medicine. "Postoperative ctDNA reflects both residual cancer and baseline tumor DNA levels. This means the test needs to be interpreted in context. A positive result after surgery may indicate higher risk, but a negative result does not always mean a patient is in the clear."
In practical terms, the blood test can provide an additional layer of insight after surgery, helping doctors better understand a patient's risk when combined with standard pathology.
Clinical context and unmet need
More than 22,000 people are diagnosed with HPV-associated throat cancer each year, according to the Centers for Disease Control and Prevention, with rates higher in men than in women. While this type of cancer responds well to treatment, patient quality of life is significantly impacted by radiation and chemotherapy. Following surgery and radiation, patients may experience long-term side effects such as difficulty swallowing, dry mouth, taste changes, sleep apnea, and hypothyroidism.
"We know that more than 90% of throat cancer cases are caused by HPV," Haring said. "Improved biomarkers could help us better tailor treatment to reduce unnecessary side effects while ensuring patients receive the therapy they need."
Future research from the Ohio State team will focus on improving the sensitivity of ctDNA testing and integrating it into multifactorial risk models that combine biomarker data with traditional clinical and pathologic risk factors to better personalize care.
