University of Edinburgh Researchers Identify Key Cellular Mechanism Driving Fibrosis in Crohn's Disease
核心洞察
University of Edinburgh researchers have identified clusters of immune cells called Crohn's lymphoid aggregates (搜索) that stimulate surrounding cells to produce excessive scar tissue in the intestines.
The study used single-cell RNA sequencing to reveal previously unrecognized interactions between immune cells, endothelial cells (搜索), and collagen-producing cells that actively promote fibrosis (搜索).
Current treatments primarily target inflammation rather than scarring itself, making this discovery crucial for developing therapies to prevent or slow fibrosis (搜索) progression.
University of Edinburgh-led researchers have made a significant breakthrough in understanding the cellular mechanisms driving fibrosis (搜索) in Crohn's disease (搜索), identifying specific immune cell clusters that stimulate excessive scar tissue formation in the intestines. The findings, published in The Journal of Pathology, offer new hope for developing targeted therapies to prevent or slow this debilitating complication.
Novel Cellular Interactions Drive Scarring Process
The research team discovered that clusters of immune cells within the gut, known as Crohn's lymphoid aggregates (搜索), stimulate surrounding cells to produce excessive scar tissue, or fibrosis (搜索). Using cutting-edge single-cell RNA sequencing technology, scientists identified a previously unrecognized link between these immune cell clusters and groups of endothelial cells (搜索) that normally line blood vessels.
"Our findings highlight previously unrecognised interactions between immune cells, endothelial cells (搜索) and collagen-producing cells in Crohn's disease (搜索)," said Dr. Michael Glinka, research fellow at the University of Edinburgh. "By combining traditional pathology with single-cell transcriptomics, we were able to confirm these changes using two independent approaches and uncover biological signalling pathways that may provide new therapeutic targets."
The endothelial cells (搜索) were found to form distinctive structures surrounding the Crohn's lymphoid aggregates (搜索), with further analysis revealing signaling interactions between these clusters and nearby cells responsible for producing collagen, suggesting they actively promote fibrosis (搜索).
Addressing Critical Treatment Gap
Crohn's disease (搜索) affects the digestive tract and involves persistent inflammation leading to fibrosis (搜索), in which excess collagen builds up in the bowel wall. This scarring can narrow and block the intestine, often requiring surgical intervention. Current treatments primarily target inflammation rather than addressing the scarring itself, representing a significant unmet medical need.
"Fibrosis (搜索) remains one of the most challenging complications of Crohn's disease (搜索) because current treatments primarily target inflammation rather than the scarring itself," explained Dr. Shahida Din, consultant gastroenterologist at NHS Lothian and honorary senior clinical lecturer at the University of Edinburgh. "Understanding the cellular signalling pathways that link immune activity to collagen production could help guide the development of therapies aimed at preventing or slowing fibrosis."
Comprehensive Tissue Analysis Reveals Key Insights
The research team analyzed intestinal tissue samples from Crohn's disease (搜索) patients with fibrosis (搜索), focusing on the ileum – the final part of the small intestine where the disease most commonly develops. Using both archived and fresh intestinal tissue samples, researchers examined structural changes across different layers of the bowel wall.
The analysis revealed significantly increased fibrosis (搜索) and immune cell infiltration in Crohn's disease (搜索) tissue compared with normal tissue. Notably, the submucosa – a deeper layer of the bowel wall – showed particularly high levels of scarring, indicating it may play an important role in the early stages of fibrosis development.
Patient Impact and Future Implications
The research findings resonate strongly with patients who have experienced the devastating effects of fibrosis (搜索). Maureen Dalgleish, a 65-year-old retired primary school teacher who contributed tissue samples to the study, has undergone four surgeries since her 1988 diagnosis to manage fibrosis-related complications.
"The idea of having medication to control or stop the fibrosis (搜索) would be amazing," said Dalgleish, who has experienced severe symptoms including abdominal pain, spasms, nausea, fever, and dizziness. "Although I realise it probably won't benefit me personally, this research could potentially be a complete game-changer for others like me."
Catherine Winsor, director of service, research and evidence at Crohn's & Colitis UK (搜索), emphasized the significance of these findings: "People who live with Crohn's often tell us how much fibrosis (搜索) and scarring can affect their lives, yet it's something current treatments don't address. This early research is really exciting because it helps us to understand what drives that scarring and where new treatments could make a difference."
The research was conducted through a UK-wide collaboration of researchers and clinicians, supported by funding from the Leona M and Harry B Helmsley Charitable Trust (搜索). Researchers are optimistic that these findings will help pinpoint therapeutic targets, paving the way for new treatments to interrupt scarring and directly address fibrosis (搜索) in Crohn's disease (搜索) patients.
