Study Design and Patient Population
The international research team, led by investigators from the University of Nottingham, analyzed routinely collected primary care data from the Clinical Practice Research Datalink Aurum, linked to hospitalization and mortality records in England. The study included adults aged 18 years or older with newly diagnosed gout (搜索), pretreatment serum urate (搜索) levels above 6 mg/dL, and first prescriptions for urate-lowering therapy (搜索) between 2007 and 2021.
The cohort had a mean age of 62.9 years, with 22.2% female patients and a mean gout (搜索) duration of 2.5 years before treatment initiation. Nearly all patients (99.2%) were prescribed allopurinol as their urate-lowering therapy (搜索).
Treatment Targets and Outcomes
Patients were stratified into two groups based on their response to treatment: those who achieved serum urate (搜索) levels below 6 mg/dL within 12 months (treat-to-target group) and those who did not (non-treat-to-target group). Only 27.3% of patients successfully reached the target within the specified timeframe.
Over a mean follow-up period of 3.45 years, 13.8% of patients experienced major adverse cardiovascular events (搜索), defined as non-fatal myocardial infarction (搜索), non-fatal stroke (搜索), or cardiovascular death.
Cardiovascular Risk Reduction
After adjusting for confounding factors, patients who achieved the target serum urate (搜索) level demonstrated superior cardiovascular outcomes. The treat-to-target group showed higher 5-year event-free survival rates compared to the non-target group (89.4% vs 88.3%), representing an absolute survival difference of 1.0%.
"Each gout (搜索) flare — including the first gout flare — is associated with a short-term increase in the risk for heart attack and stroke (搜索)," said Abhishek, PhD, professor of rheumatology at the University of Nottingham and lead researcher. "It is important to find out if urate-lowering treatment that achieves a blood urate level less than 6 mg/dL can also prevent heart attack and stroke in people with gout."
The cardiovascular benefit was particularly pronounced in specific patient subgroups. Patients with high or very high baseline cardiovascular risk profiles showed stronger associations between achieving target urate levels and reduced cardiovascular events. Most notably, patients who reached even more stringent serum urate (搜索) levels below 5 mg/dL demonstrated the greatest benefit, with a 23% relative risk reduction for major adverse cardiovascular events (搜索) and a 2.6% absolute survival benefit.
Clinical Implications
The study findings provide compelling evidence for the cardiovascular benefits of treat-to-target urate-lowering strategies in gout (搜索) management. The research team noted that fewer gout flares were also observed in the treat-to-target group, suggesting multiple mechanisms may contribute to the observed cardiovascular protection.
"The findings of this study provide another potential benefit of treat-to-target ULT for people with gout (搜索)," Abhishek explained. "Given the high cardiometabolic burden of people with gout, the findings of this study provide an additional reason for treating gout to target blood urate level. These findings should be considered by specialist societies when they update their recommendations."
Study Limitations and Future Directions
The researchers acknowledged several limitations of their observational study design, including potential residual confounding, incomplete reporting of gout (搜索) flares, and possible misclassification of serum urate (搜索) exposure. However, extensive statistical adjustments, sensitivity analyses, and the use of negative control outcomes supported the internal validity of their findings.
The authors have called for additional research to clarify the biological mechanisms underlying the observed association between urate-lowering therapy (搜索) and cardiovascular risk reduction.
