Vision-Preserving Uveal Melanoma Treatment Shows Unexpected Survival Benefits in UCLA Study

A retrospective study from UCLA has revealed that a vision-preserving surgical technique for uveal melanoma (搜索) may provide unexpected survival benefits beyond its intended purpose of protecting patients' eyesight. The research, published in Cancers, analyzed 37 patients treated by a single vitreoretinal surgical oncologist and found remarkably low metastasis rates compared to historical data.
At a median follow-up of 4.0 years, the 5-year distant metastasis-free survival (DMFS) rate reached 80%, with the median DMFS not yet reached. Only 16.2% (n = 6) of patients developed metastatic disease (搜索), and the study noted no intraocular treatment failures. Most significantly, the overall survival rate remained 100% at 5 years, with only one patient dying from metastatic disease prior to analysis.
Treatment Approach and Patient Demographics
The majority of patients (73%) received plaque brachytherapy (搜索) combined with vitrectomy and silicone oil (搜索) for radiation attenuation, while 19% underwent brachytherapy alone and 8% received enucleation. The median patient age was 59 years, with 70% being male and 90% White and non-Hispanic.
"The radiation plaque goes on the external part of the eye to kill the tumor. However, when we do the radiation like that, we can kill the melanoma, but what happens is that the radiation also affects healthy, non-tumor tissue in the eye," explained Tara A. McCannel, MD, PhD, director of the Ophthalmic Oncology Center at the UCLA Stein Eye Institute (搜索). "We take out the entire vitreous humor, and we fill it with a denser substance. In our case, we use silicone oil (搜索). We [now] have a denser substance in the eye. The radiation will still kill the tumor, but it is attenuated or shielded, so it limits the radiation from affecting healthy tissue beyond the tumor."
Tumor Staging and Outcomes
The study included patients across all tumor stages: 40.5% had T1 tumors, 24.3% had T2, 21.6% had T3, and 13.5% had T4. The average progression-free survival varied by stage, with T1 patients achieving 6.41 years ± 4.18 years, T2 patients 3.17 years ± 1.56 years, T3 patients 4.37 years ± 4.03 years, and T4 patients 1.19 years ± 0.73 years.
Comparison to Historical Data
The results significantly exceeded expectations based on established literature. "In terms of survival, our 5-year estimated survival was 100% in this cohort, whereas the Collaborative Ocular Melanoma Study for medium tumors [showed an] OS or metastasis rate [of about] 80%. The larger tumor group [experienced a] 60% OS rate," McCannel noted. "Our numbers were very different. This is a small study, and it's a small sample, but even in a small cohort, we were still surprised to see our results not fall in line and be close to what we know in the reported literature."
Risk Assessment and Monitoring
Patients underwent cytogenetic and genomic testing using fine-needle aspiration, with high-risk patients defined as having monosomy 3 (搜索) or Class 2 GEP (搜索) scores, while low-risk patients had Disomy 3 or Class 1A or 1B GEP scores. The study found that cytogenetic and genomic indicators of low risk for metastasis had a negative predictive value higher than 90%. However, most patients with high-risk factors did not develop metastases, prompting calls for more accurate testing methods.
Clinical Implications and Future Research
"Everything about uveal melanoma (搜索) has always been painted with limited hope, with patients expected to lose vision and eventually die once the cancer spreads," McCannel stated. "We know that this unique surgical technique clearly saves vision. However, we did not expect the technique to alter metastasis and mortality. And anything that improves survival in uveal melanoma is a game-changer."
The researchers acknowledge that further investigation is needed to understand the mechanism behind these improved outcomes. "Although we observed a low metastatic rate in patients treated with plaque brachytherapy (搜索), vitrectomy, and silicone oil (搜索) placement, it is not clear how these procedures may have resulted in improved outcomes," the authors noted. "Further laboratory and more rigorous clinical investigations are needed to evaluate whether vitrectomy and silicone oil exert tumoricidal or immunomodulatory effects in a manner that impacts systemic progression or whether more effective brachytherapy delivery alone is responsible for the observed outcome."
The findings suggest that the combination approach may offer both vision preservation and survival benefits, potentially changing the treatment paradigm for uveal melanoma (搜索) patients. McCannel emphasized that "since most centers still rely on plaque brachytherapy (搜索) alone to treat uveal melanoma, these findings underscore the need to adopt our vitrectomy with silicone oil (搜索) approach."
