Voice Disorders May Be an Earlier, Stronger Sign of Dementia Than Hearing Loss, Large Study Finds

Changes in the way a person speaks could be an early warning sign of cognitive decline, according to a large new study that found voice disorders may be more strongly linked to future dementia (搜索) risk than hearing loss (搜索) alone. Researchers at Drexel University's College of Medicine analyzed data from 833,417 patients, tracking people after they were diagnosed with either a voice disorder, hearing loss, both conditions, or neither, and then examined who later developed cognitive impairment (搜索) or dementia. The findings were published in the Journal of Voice.
The results revealed a striking pattern: people with a diagnosed voice disorder but no hearing loss (搜索) were 58 percent more likely to develop cognitive impairment (搜索) or dementia (搜索) than people with neither condition. By comparison, those with hearing loss alone faced a 27 percent higher risk. The strongest association was seen among people who experienced both a voice disorder and hearing loss.
Voice disorders included symptoms such as changes in pitch, reduced speech clarity, altered volume, reduced vocal range, pain when speaking or singing, and other medically diagnosed vocal conditions.
A Challenge to the Prevailing Focus on Hearing Loss
The research builds on previous evidence linking hearing loss (搜索) and dementia (搜索). Hearing loss is already considered the largest modifiable risk factor for dementia, and the team's data supported this, reporting that hearing loss was associated with a 27% increased risk of cognitive decline compared to matched controls. However, the new findings suggest voice problems may serve as an even earlier indicator in some cases.
"Our work shows that voice disorders may be an earlier and stronger sign of cognitive decline than hearing loss (搜索) alone," said senior author Robert Sataloff, MD, a professor and chair in Drexel's College of Medicine. "We found the strongest link with dementia (搜索) among patients experiencing a voice disorder and hearing loss; but importantly, we found that those with a voice disorder and no hearing loss were at greater risk than patients who experience hearing loss only."
The findings challenge hearing loss (搜索) as the prevailing area of focus for diagnosis of cognitive decline. At the same time, the data in the study did not contradict earlier studies linking hearing loss and dementia (搜索), including a study in The Lancet which found that counseling and hearing aid use reduced a three-year cognitive change in older adults who were at increased risk for cognitive decline.
The Biological Basis for Voice as an Early Signal
Dr. Robert Sataloff, the study's senior author, said reduced communication caused by voice disorders could play a role. "If you have a voice disorder that impacts how your voice sounds or if it's painful when you attempt to talk or sing, it's only natural to participate less often in brain-supporting social or other engaging activities," Sataloff said.
The researchers noted that social interaction and mentally stimulating activities are known to support cognitive health. If speaking becomes uncomfortable or difficult, people may gradually withdraw from conversations, group activities, or hobbies that help keep the brain engaged.
Sataloff also pointed to Parkinson's disease (搜索) as an example of how voice changes can emerge before more obvious symptoms appear. "We already see this in patients with Parkinson's disease, in which voice disorders often precede other physical signs of the disease," he said, raising the possibility that vocal changes could serve as an early neurological warning sign in some people.
Dr. Chandril Chugh, a neurologist who was not involved in the research, told Newsweek that the connection between voice and cognition is biologically plausible because both depend on many of the same neural systems. "Speech production depends on a tightly coordinated network involving the frontal and temporal lobes, basal ganglia, cerebellum, and brainstem," Chugh said.
He explained that these regions are often affected early in neurodegenerative diseases, which may allow subtle vocal changes to emerge before memory symptoms become noticeable. "When these networks start to break down, subtle vocal changes like reduced volume, a flatter or more monotone pitch, or a strained, breathy quality can show up years before more obvious symptoms like memory loss appear," he added.
"Voice is really a real-time readout of very complex neural coordination, so when that coordination starts slipping, the voice often shows it early," Chugh said.
Implications for Screening and Clinical Practice
The findings could eventually have practical implications for screening, particularly because voice monitoring is relatively simple and inexpensive. However, experts caution that vocal changes alone should not be viewed as proof that someone is developing dementia (搜索).
"I think this is a genuinely promising direction, and it makes sense as a low-cost, non-invasive screening tool," Chugh said. "But voice changes on their own aren't specific to dementia (搜索)."
He noted that similar symptoms can also be caused by Parkinson's disease (搜索), depression, vocal cord disorders, or normal aging. Instead, he said, voice changes should be viewed as a signal that further evaluation may be needed. "In practice, I'd treat a voice change as a flag that prompts a proper cognitive workup, not as a diagnosis by itself," Chugh concluded.
The study's authors argue that greater attention should be paid to voice disorders during medical evaluations. "It may be valuable for many of these patients to talk with an otolaryngologist who can arrange a cognitive assessment to help patients develop an appropriate care plan," Sataloff said. They suggest that patients experiencing persistent voice problems may benefit from seeing an otolaryngologist, an ear, nose and throat specialist, who could recommend cognitive assessment when appropriate.
Disease Burden and Study Limitations
Roughly 17.9 million U.S. adults report having had a voice problem within the past 12 months, and 9.4 million of them have had an issue that lasted for a week or longer, according to the National Institutes of Health. The findings are particularly important for the four in ten adults who experienced a voice disorder and did not pursue medical care, who may be at higher risk of cognitive decline.
More than six million Americans have Alzheimer's or another form of dementia (搜索), a number that is expected to more than double by 2060, according to the National Institutes of Health. The findings are also relevant to the more than four in 10 Americans over 55 who will receive a dementia diagnosis in their lifetime. Early diagnosis empowers a patient to develop more robust plans for taking care of finances and prepare for future health issues to ensure the best possible quality of life.
The researchers emphasized that the study identified an association, not a cause-and-effect relationship. The team also found that those with a voice disorder and hard of hearing were older and experienced higher rates of health issues like high blood pressure, heart disease and Type 2 diabetes. More research is needed to determine whether specific voice disorders are more strongly tied to dementia (搜索) risk and whether treating vocal problems could help slow cognitive decline in the future.
