The findings raise important questions about the adequacy of a one-size-fits-all approach to diabetes prevention and point toward the need for personalized strategies targeting specific metabolic vulnerabilities.
Six Distinct Risk Clusters Redefine Prevention
Several years ago, DZD researchers stratified people at risk of type 2 diabetes (搜索) into six clearly defined clusters, each differing considerably in how often diabetes develops and how diabetes-related complications progress. Among these, clusters 3 and 5 were identified as having an especially high likelihood of progressing to type 2 diabetes.
The latest analysis examined whether long-term weight loss and lifestyle-based diabetes prevention produced different results across these risk groups, drawing on data from the Tübingen Lifestyle Intervention Program (TULIP).
Sustained Weight Loss, Persistent Risk
The TULIP study enrolled participants at increased risk of type 2 diabetes (搜索) in a two-year lifestyle program emphasizing healthy eating, weight loss, and increased physical activity. Participants were then monitored for approximately nine years. The current analysis concentrated specifically on those who achieved substantial weight loss and successfully maintained it over the long term.
"We were particularly interested in whether individuals in risk clusters 3 and 5 differed from those in other clusters with regard to improvements in blood glucose levels and the prevention of type 2 diabetes (搜索)," explained Professor Norbert Stefan, the lead author of the study.
The results were striking. "We were very surprised to find that, despite a large and sustained weight loss of 8% and after a very long follow-up period of 9 years, individuals in risk cluster 5 showed increasing blood glucose levels, declining insulin secretion, and a persistently high risk of type 2 diabetes (搜索)," Stefan said.
In contrast, participants in other risk groups generally experienced greater metabolic benefits from the same lifestyle changes, suggesting that cluster 5 represents a biologically distinct high-risk profile that resists conventional prevention strategies.
Fatty Liver and Insulin Resistance as Key Drivers
To understand why lifestyle intervention appeared to provide less protection for people in cluster 5, the researchers investigated the underlying biological mechanisms. Their analysis pointed to insulin resistance as a major contributing factor, most likely linked to severe fatty liver disease (搜索).
Fat accumulation in the liver may impair the ability of pancreatic beta cells to release sufficient insulin, causing blood sugar levels to continue rising even after meaningful weight loss. These findings support earlier evidence showing that fatty liver disease (搜索) and insulin resistance are the predominant disease processes affecting individuals in Tübingen's risk cluster 5, leaving them especially vulnerable to both type 2 diabetes (搜索) and cardiovascular disease.
Toward Personalized Diabetes Prevention
The results indicate that people in cluster 5 may not receive the same metabolic benefits from lifestyle intervention as those in other risk groups, even when they achieve substantial and lasting weight loss—particularly with respect to blood sugar regulation.
If future prospective research confirms these findings, diabetes prevention may need to become more personalized. Individuals with high-risk biological profiles such as cluster 5 may require more intensive care or treatments specifically designed to address their metabolic problems, including therapies that target insulin resistance and fatty liver disease (搜索) alongside lifestyle modification.
