A comprehensive WHO-supported cohort study conducted across five Egyptian university hospitals has provided new insights into COVID-19 (搜索) vaccine effectiveness among healthcare workers during the Omicron (搜索)-dominant period. The research, spanning one year of follow-up from July 2022 to August 2023, offers critical real-world evidence for vaccine policy decisions in the Eastern Mediterranean region.
Study Design and Population
The prospective cohort study enrolled 1,230 healthcare workers from Al-Azhar University hospitals across three Egyptian governorates. Participants included physicians, nurses, laboratory technicians, and administrative staff who interact with patients or handle potentially infectious materials. The study achieved remarkably high vaccination coverage, with 91.4% of participants having received at least one COVID-19 (搜索) vaccine dose.
The vaccination landscape among participants reflected Egypt's diverse vaccine portfolio. BBIB-CorV (搜索) or WIBP-CorV (搜索) (Sinopharm) was the most commonly used vaccine at 38.5%, followed by ChAdOx1 nCoV-19 (AstraZeneca) at 31.3%. Other vaccines included BNT162b2 mRNA (Pfizer/BioNTech), mRNA-1273 (Moderna), CoronaVac (Sinovac (搜索)), and Ad26.COV2.S (Janssen).
Vaccine Effectiveness Results
The study's primary endpoint was symptomatic RT-PCR confirmed SARS-CoV-2 (搜索) infection. Among fully vaccinated healthcare workers, the adjusted vaccine effectiveness was 68% (95% CI −28–92%) compared to unvaccinated participants. For those who received booster doses, effectiveness was 64% (95% CI −170–95%). However, these results did not reach statistical significance, likely due to the limited number of infection events during the study period.
During follow-up, 232 healthcare workers developed COVID-19 (搜索)-like symptoms, but only 108 agreed to provide nasopharyngeal samples for testing. Of these, 18 tested positive for SARS-CoV-2 (搜索) RNA (16.7%), representing an incidence of 0.44 cases per 10,000 person-days. Notably, 15 of the 18 positive cases (83.3%) occurred in vaccinated individuals, yet none required hospitalization or died.
Hybrid Immunity Shows Enhanced Protection
A particularly significant finding emerged when analyzing participants with hybrid immunity—those who had both vaccination and prior infection. The study revealed that all unvaccinated healthcare workers were seropositive at enrollment, indicating universal prior infection in this group. In contrast, prior infection rates were 11.8% among partially vaccinated workers, 18.2% among fully vaccinated, and 18% among those with booster doses.
Vaccine Type Comparisons
When examining effectiveness by vaccine type, mRNA vaccines appeared superior, with no breakthrough infections recorded among healthcare workers who received BNT162b2 mRNA (Pfizer) or mRNA-1273 (Moderna). The adjusted vaccine effectiveness for ChAdOx1 nCoV-19 (AstraZeneca) was 85% (95% CI −19–98%), while BBIB-CorV (搜索) or WIBP-CorV (搜索) (Sinopharm) showed 65% effectiveness (95% CI −59–92%). Due to limited events, effectiveness for CoronaVac (Sinovac (搜索)) could not be calculated.
Serological Findings and Antibody Responses
At enrollment, 99.8% of study participants were seropositive for COVID-19 (搜索) anti-spike-binding antibodies, with levels exceeding 2,500 U/ml in 53.3% of healthcare workers. Only three participants (0.3%) had negative binding antibodies despite completing their vaccination series. This near-universal seropositivity reflects the high exposure risk faced by healthcare workers and the effectiveness of vaccination programs in generating immune responses.
Timing and Waning Immunity
The median duration from receiving the second vaccine dose to study end was 631 days, while the median time from booster dose to study end was 394 days. All breakthrough infections among two-dose vaccinated participants occurred in those who were more than 365 days from their second dose, with an adjusted vaccine effectiveness of 68% (95% CI −25–92%) in this group.
Omicron Era Context
The study was conducted during Egypt's fifth COVID-19 (搜索) wave, when the Omicron (搜索) variant was dominant.
