CASSANDRA Phase 3 Trial Shows PAXG Superior to mFOLFIRINOX in Preoperative Pancreatic Cancer Treatment

- The CASSANDRA phase 3 trial demonstrated that preoperative PAXG chemotherapy significantly improved event-free survival compared to mFOLFIRINOX in resectable and borderline resectable pancreatic cancer patients. - PAXG achieved a median event-free survival of 16.0 months versus 10.2 months with mFOLFIRINOX, with three-year event-free survival rates of 33% versus 13% respectively. - The four-drug PAXG regimen showed superior disease control rates, stronger biomarker responses, and more favorable pathological downstaging without increased severe toxicity. - These findings establish preoperative PAXG as a new standard treatment option and preferred comparator for future neoadjuvant trials in pancreatic ductal adenocarcinoma.
