- The TIME-HF trial evaluated a nurse-led, mobile health-integrated intervention to improve uptake of guideline-directed medical therapies in heart failure patients in India. - The intervention significantly increased days alive and out of hospital, with a higher probability of surviving two years without hospitalisation (84.0% vs. 79.4%; p<0.001). - The nurse-coordinated model was associated with a 22% reduction in deaths at two years compared with usual care (p=0.028). - Adherence to all four guideline-directed medical therapies reached 37.3% in the intervention group versus 22.1% with usual care, suggesting a scalable blueprint for under-resourced settings.
