Patients admitted to the intensive care unit who meet the following criteria
1. On a ventilator for at least 72 hours
2. Developed delirium while in the ICU
3. Unscheduled admission and ICU stay of 72 hours or more
The following patients who are thought to have functional impairment other than sequelae associated with intensive care.
1. patients who, at the time of discharge, are in almost the same condition as when they entered the ICU. 2. Patients who have been transferred from active treatment to palliative treatment. 3. patients with a history of cognitive impairment prior to ICU admission. 4. Patients with neurological disorders. 5. Residents of long term care facilities. 6. Other patients deemed inappropriate by the attending physician or nurse.
