65-85 years old
admitted in all wards of hospital excepted ICU
patients was not high risk based on MAPLe algorithm
speak with persian language

Name Time Method Functional, clinical and quality of life indicator. Timepoint: At the beginning of the study, at home visit, at the time of phone call, 30 days after the intervention began. Method of measurement: based on Minmum Data Set -Home Care indicators.
- Secondary Outcome Measures
Name Time Method Pain assessment. Timepoint: During initial assessment, home visit and phone calls. Method of measurement: Using the Pain Tool, from MDS-HC.;Cognition assessment. Timepoint: During the initial evaluation and final evaluation. Method of measurement: Using the Cognition Tool, from MDS-HC.
