- •Spirometry confirms COPD
- •At least 10 pack year smoking history

- •Acute coronary syndrome
- •Already treated with beta-blocker
- •Major contra-indication to cardio-selective beta-blocker
- •Terminal illness
- •Primary admission diagnosis of pneumonia or other acute respiratory disease
