- Brief Summary
*Bhagandara* is considered under the heading of eight major diseases*(Ashtamahagada)* in Ayurveda and as per *Sushruta*, all types of *Bhagandara*are difficult to treat. It can be compared with Fistula-in-ano in modernparlance. It is a chronic disease, though not fatal but posing quite discomfortand trouble to the patient. Despite of much advancements in medical science, itis still considered as a big challenge to the surgeons as there is no anysatisfactory treatment modality is available so far in modern surgery is concerned.
*Ksharasutra*treatment is considered as successful minimum invasive surgicalprocedure for treatment of fistula in ano. It is such a simple and safe remedyfor anal fistula and it is becoming universally acceptable day by day. *Ksharasutra*treatment heals the fistulous tract with the integrity of sphincter’smechanism. *Apamarga* (*Achyranthes aspera) Ksharasutra*is wellproven to be an effective treatment for fistula-in-ano and has beenstandardized by Central Council for Research in Ayurvedic Sciences (CCRAS) anapex research organization of the Government of India.
The prevalence of anal fistula in India varies from 17% to20%, while in a London hospital approximately 10% of all patients and 4% of newpatients of ano-rectal disease were reported to suffer from this disease amongthe all ano-rectal disorders.
*Ksharasutra*in situ and by virtue of its caustic effect providescontinue drainage of the abscess cavity or fistula track. It removes unhealthytissues and promotes healthy granulation tissue, enhancement of fibrolysis,separation of debris through the fistulous track, removal of debris andcleansing wound.
Bacteriology of anal fistula has attractedconsiderable interest over the past years. The major point of interest has beenthe finding that the presence of gut bacteria, as opposed to bacteria derivedfrom skin, in the pus from anal abscesses that indicates an unsuspected analfistula. Whether this is because tracks that connect with the ano rectum aremore likely to be colonized by gut bacteria that virulent of unusual gutorganisms are important as etiological factors in the genesis of anal fistulahas never been investigated. During the *Ksharasutra*treatment noantibiotic or antimicrobial drug is administered to the patients. But thechronic granulating tract which usually occurs in anorectal region frequentlygets contaminated with fecal matter and harbour the microorganism. So far noresearch has been directed to assess antimicrobial activity of *Ksharasutra*,but it is compulsory to study this aspect of the treatment to justify it as abetter alternative to the conventional surgical methods. Considering this, thecurrent study has been initiated.

- Patients having classical signs and symptoms of Bhagandara (Fistula-In-Ano) 2.
- Presence of abnormal opening in perianal region.
- Recurrent muco-purulent discharge from abnormal opening present in perianal region.
- Recurrent perianal pain/ discomfort.
- Bhagandara associated with one or more of the following conditions: 1.
- Uncontrolled tuberculosis 2.
- Uncontrolled Hypertension.
- Uncontrolled diabetes mellitus.
- Osteomyelitis.
- Chronic or acute ulcerative colitis.
- Crohn’s disease.
- Any malignancy.
- Venereal disease, HIV and HBsAg positive cases.
- Fistula other than ano-rectal.
