Inclusion Criteria

  • age more than 18 years
  • respiratory rate more than 24 breath per minuts
  • use of accessory muscle or abdominal paradox
  • room air oxygen saturation below 95 %

Exclusion Criteria

  • Cardiac arrest
  • Unstable medical condition e.g. shock myocardial infarction
  • Respiratory failure defined by respiratory rate > 35 breath per minutes and oxygen saturation < 90% after oxygen theary
  • High risk for aspiration
  • Large amount of secretion
  • Glasgow coma scale < 13
  • unco-operated patients
  • untreated pneumothorax
  • recent airway or upper GI tract or esophageal surgery
  • required immediate intubation
