Inclusion Criteria
Multimorbidity, i.e. the presence of at least two diseases requiring treatment,
Age = 70 years,
Stay in one of the participating clinics for in-patient treatment,
The patient's ability to consent,
written informed consent of the patient to participate in the study
Exclusion Criteria
Age < 70 years,
There is no illness or only one illness requiring treatment,
no declaration of consent to participate in the study,
Presence of a language barrier,
Insufficient compliance (with uncontrolled alcoholism, drug addiction),
patient incapable of consent,
other reasons at the discretion of the investigator.
