Eligibility Criteria

Inclusion Criteria

  • Inclusion criteria for patients with stroke
  • 1\. Have age 18 years or above
  • 2\.Have ischemic stroke
  • 3\.Be discharged from hospital within 10 Days
  • 4\.Have severity of stroke score ( within 16 to 42\) by using national institutes of health stroke scale
  • Inclusion Criteria for Caregivers: Caregivers who will be
  • 1\. 18 years or above
  • 2\.The primary caregiver at home.
  • 5\.Be able to understand Hindi
  • 6\.Be ready to become part of the research

Exclusion Criteria

  • 1\.Unconsciousness with GCS \> 8
  • 2\.Having complications of diabetes mellitus such as neuropathy and diabetic foot
  • 3\.Having existing cerebro\-vascular conditions with physical disability like limb function
  • 4\.Having mental illness (anxiety disorder, post traumatic stress condition, psychosis, schizoaffective \& schizophrenia illness)
  • 5\.Already having cognitive dysfunction (depression and delirium) and physical dysfunction before having stroke
