•Inclusion criteria for patients with stroke
•1. Have age 18 years or above
•2.Have ischemic stroke
•3.Be discharged from hospital within 10 Days
•4.Have severity of stroke score ( within 16 to 42) by using national institutes of health stroke scale
•Inclusion Criteria for Caregivers: Caregivers who will be
•1. 18 years or above
•2.The primary caregiver at home.
•5.Be able to understand Hindi
•6.Be ready to become part of the research

•1.Unconsciousness with GCS > 8
•2.Having complications of diabetes mellitus such as neuropathy and diabetic foot
•3.Having existing cerebro-vascular conditions with physical disability like limb function
•4.Having mental illness (anxiety disorder, post traumatic stress condition, psychosis, schizoaffective & schizophrenia illness)
•5.Already having cognitive dysfunction (depression and delirium) and physical dysfunction before having stroke
