- •1. Adult patients, diagnosed with HF within the last 12 months.
- •2. Ability to be accessed by mobile phone and internet signals.
- •3. Ability to use the mobile app either with or without assistance from a caregiver.
- •4. Having a Thai Mental State Examination (TMSE) score >23
排除标准
- •1. Having cancer, chronic kidney disease, or surgery planned within two months, and/or were on dialysis.
- •2. Submitting self-monitoring data fewer than 30 days of the total 37 days.
