Eligibility Criteria
Inclusion Criteria
- •1\. Adult patients, diagnosed with HF within the last 12 months.
- •2\. Ability to be accessed by mobile phone and internet signals.
- •3\. Ability to use the mobile app either with or without assistance from a caregiver.
- •4\. Having a Thai Mental State Examination (TMSE) score \>23
Exclusion Criteria
- •1\. Having cancer, chronic kidney disease, or surgery planned within two months, and/or were on dialysis.
- •2\. Submitting self\-monitoring data fewer than 30 days of the total 37 days.
