Inclusion Criteria
• Age =70 years
• At least two chronic medical conditions
• Regular intake of = 5 medications
• Mentally and physically able to participate in the study (as judged by a study physician or the treating physician at Agaplesion Bethanien Hospital)
Exclusion Criteria
• MMSE =23 points
• Known diagnosis of dementia
• Insufficient knowledge of written and/or spoken German
• No written informed consent
