Inclusion Criteria
Patients aged >=65 years with at least 5 chronic medication prescriptions in at least two different organ systems.
Exclusion Criteria
Patients who cannot act for themselves, patients with insufficient knowledge of Dutch language, patients with a life-expectancy of less than one year (according to the GP) and patients who already had a medication review in the last 12 months.

- Secondary Outcome Measures
Name Time Method Secondary outcomes are type and number of medication alerts, changes in medication prescription, medication adherence, drug burden index, and quality of life. Relevant outcomes will be measured at baseline (before the intervention), after completion of the intervention, and after 12 months follow-up.
