Age = 70 with multimorbidity, or = 70 80; suffering from at least one geriatric syndrome, cognitive and physical ability to perform basic self-management
Exclusion Criteria
delirium, complete dependence in daily activities

- Primary Outcome Measures
Name Time Method The primary endpoint for the cross-sectional study is self-management (Patient Activation Measure, PAM-13; und Appraisal for Self-Care Agency Scale, ASAS-R). <br>For the follow-up data collection, the impact of self-management on independence in activities of daily living will be determined.
- Secondary Outcome Measures
Name Time Method Secondary endpoints include mortality, quality of life (WHOQOL-Bref), and health service utilization during the observation period as a function of self-management.
