- Target Recruitment
- 40
All patients should fulfil these inclusion criteria:
i)Orthodontic patients requiring first upper premolars extraction and two-step retraction technique
ii)Complete permanent dentition (except third molars)
iii)No previous orthodontic treatment
iv)Healthy patients without systemic diseases that can affect bone and tooth movement
v)Good oral hygiene and healthy periodontium which will be evaluated clinically (probing depth 3 mm), with no radiographic evidence of bone loss, plaque and gingival index 1 according to Loe and Silness.
EXCLUSION CRITERIA
i)History of long term medication will be excluded because nonsteroidal anti-inflammatory drugs and hormone supplements interfere with bone metabolism.
ii)Those with unilateral chewing or any parafunctional habits, skeletal crossbite or occlusal interferences.
iii)Periodontally compromised patients because poor bone quality can affect orthodontic tooth movement and development of any mobility during the course of treatment can lead to incorrect measurement of orthodontic tooth movement.
iv)Impacted canines and canines with dilacerated roots can make orthodontic tooth movement difficult with a high possibility of root resorption.

- Primary Outcome Measures
Name Time Method The primary outcome will be to compare the rate of canine movement during distalization of maxillary canine using piezocision and low level laser therapy on completion of maxillary canine retraction on the experimental side. <br/ ><br>Timepoint: The rate of orthodontic tooth movement(OTM) at the end of 15 days (R1) will be recorded as T0 ââ?¬â?? T1 divided by 15. Similarly, the rate of orthodontic tooth movement will be calculated and recorded every 15 days for the first 3 months (R2 ââ?¬â?? R6). The rate of orthodontic tooth movement on completion of canine retraction (R7) will be recorded as T6 ââ?¬â?? T7 divided by the number of months. The R1 - R7 readings will be calculated for both the experimental and control sides and will be compared.
- Secondary Outcome Measures
Name Time Method The secondary outcome will be molar anchorage loss, gingival & plaque scores, gingival sulcus depth, mobility scores and presence/absence of gingival recession in the maxillary canine. Any complication(canine vitality & root resorption) during this study will be recorded.Timepoint: Data that will be collected at T0 (after completion of alignment and levelling: day1 of canine retraction), and T7 (on the completion of canine retraction on the experimental side PZ/LLLT) depending on the parameter.
