Inclusion Criteria
a)Patients with selected NCDs (cardiovascular diseases, diabetes mellitus & COPD) reporting to the outpatient department of selected health facilities
b) Patients who are likely to stay in the study area for at least one year
c) Those willing to take part in the study
Exclusion Criteria
Patients with other NCDs like carcinomas, psychiatric disorders, asthma

Name Time Method Change of 10-year risk of developing cardiovascular disease (CVD) using the WHO/ International Society of Hypertension (ISH) risk-assessment charts for SEAR after one year of behavioural change intervention.Timepoint: At enrollment and at 3, 6 & 12 months of enrollment

Name Time Method Changes in behavioural characteristics (e.g. Tobacco smoking, excessive alcohol intake, physical inactivity and an unhealthy diet), biological characteristics (e.g. blood pressure control, blood sugar control, weight control) and adherence to prescribed treatment as compared to the baseline. <br/ ><br> <br/ ><br>Timepoint: At enrollment and at 3, 6 & 12 months of enrollment;The burden of patients with COPD reporting to the OPDs at the selected health centers and those diagnosed with COPD on opportunistic screening.Timepoint: At enrollment
