To evaluate the effects of an eccentric isokinetic exercise program on shoulder pain, function, and ultrasonographic supraspinatus muscle architecture in patients with rotator cuff tendinopathy, and to compare outcomes with a standardized home exercise program.

Detailed Description

This study aims to evaluate the effects of eccentric isokinetic exercise on pain, shoulder function, and supraspinatus muscle architecture parameters in individuals diagnosed with rotator cuff tendinopathy. The study's primary hypothesis is that controlled eccentric isokinetic loading will induce measurable architectural adaptations in the supraspinatus muscle, in addition to clinical improvement.

Inclusion Criteria

  • Being between the ages of 20 and 50
  • Having a diagnosis of rotator cuff tendinopathy based on medical history, physical examination, and MRI findings
  • Having full shoulder joint range of motion
  • Absence of upper extremity deformity that would interfere with ultrasonographic measurement
  • Having provided written informed consent

Exclusion Criteria

  • Glenohumeral instability
  • Adhesive capsulitis (stages 2-3)
  • Radiographic evidence of calcific tendinitis or significant bursitis •Cervical radiculopathy Upper extremity or spinal deformities
  • History of surgery involving the shoulder, cervical spine, or back
  • Neurological or systemic disease (rheumatological, metabolic, etc.)
  • Physical therapy for the shoulder, neck, or back region within the last 6 months
  • Corticosteroid injection into the affected shoulder within the last 6 months
  • Exposure to intensive repetitive shoulder movements in sports or occupational activities
  • Use of medications that could affect exercise performance or metabolic response

Outcomes

Primary Outcomes

To evaluate the effects of an eccentric isokinetic exercise program on shoulder compare outcomes

Time Frame: 6 mounth

To evaluate the effects of an eccentric isokinetic exercise program on shoulder pain, function, and ultrasonographic supraspinatus muscle architecture in patients with rotator cuff tendinopathy, and to compare outcomes with a standardized pain was assessed using VAS. The VAS is an ordinal scale that rates pain from 0 to 10. Low scores indicate low pain, while high scores indicate severe pain.

Western Ontario Rotator Cuff Index

Time Frame: 3 months

It assesses the impact of shoulder pain on five key domains: physical symptoms, sports and recreation, work function, lifestyle, and emotional well-being. The questionnaire takes less than 10 minutes to complete, features 21 items, and is scored out of 2100 points, with higher scores indicating greater dysfunction.

Ultrasonographic measurement

Time Frame: 3 months

Supraspinatus muscle thickness was measured in millimeters using ultrasonography. High values indicate good function.
