Eligibility Criteria
Inclusion Criteria
- •1 Adults with diabetes registered and regularly attending (at least once a month) the NCD clinics of the study setting.
- •2 All Junior Public Health Nurses in charge of the subcentres under study.
Exclusion Criteria
- •1\.Not willing to give consent for participation.
- •2\.Presence of other comorbidities like malignancy, renal or hepatic disease.
- •3\.Pregnant women.
- •4\.Inability to perform physical activity like bedridden patients, history of a cardiac event in past 12 months.
- •5\.Cognitive impairment, dementia or mental health problems.
- •1\)Not willing to give consent for participation
