入选标准
- •1 Adults with diabetes registered and regularly attending (at least once a month) the NCD clinics of the study setting.
- •2 All Junior Public Health Nurses in charge of the subcentres under study.
排除标准
- •1.Not willing to give consent for participation.
- •2.Presence of other comorbidities like malignancy, renal or hepatic disease.
- •3.Pregnant women.
- •4.Inability to perform physical activity like bedridden patients, history of a cardiac event in past 12 months.
- •5.Cognitive impairment, dementia or mental health problems.
- •1)Not willing to give consent for participation
