Inclusion Criteria
1 Adults with diabetes registered and regularly attending (at least once a month) the NCD clinics of the study setting.
2 All Junior Public Health Nurses in charge of the subcentres under study.
Exclusion Criteria
Patients
1.Not willing to give consent for participation.
2.Presence of other comorbidities like malignancy, renal or hepatic disease.
3.Pregnant women.
4.Inability to perform physical activity like bedridden patients, history of a cardiac event in past 12 months.
5.Cognitive impairment, dementia or mental health problems.
