Brief Summary

Obesity is an alternate cause of death worldwide, with increasing prevalence in adults and children. It is one of the most serious public health problems of the 21st century. Though modern system of medicine has its own therapeutic modalities in tackling obesity, they are very commonly associated with many adverse effects. Surgery done for fat removal has so many temporary and long term complications like Bleeding, Internal hernia, Hair loss etc. Considering this, the global population is enthusiastically looking towards effective natural remedies.

Thus, Obesity is a serious problem and a serious solution is needed to find out. So this disease is selected for research work.

ï‚§In this study, total 30 patients were registered and all patients had completed treatment along with follow-up.

ï‚§36.67% of patients belonged from age group of 18-30 years and 41-60 years

ï‚§80% of patients were female and 66.67% were Hindu

ï‚§36.67% of patients were graduate while 43.33% were housewives

ï‚§43.33% of patients belonged to middle-class backgrounds

ï‚§100% of patients belonged to urban areas

ï‚§80% of patients were married

ï‚§56.67% of patients were vegetarian

ï‚§56.67% of patients were having Vishamagni

ï‚§70% of patients had Madhyama Koshtha

ï‚§93.33% of patients had history of onset of disease in adolescence

ï‚§83.33% of patients had progressive type of course of disease

Reviewing Nidana of Sthaulya-

ï‚§96.67% of patients had a Madhura Rasa dominant diet while 96.67% had Guru Guna dominance in diet

ï‚§90% of patients were found to intake water after meal while 56.67% patients were found to have taken meal irregularly

ï‚§63.33% of patients were eating in between meals frequently

ï‚§60% of patients had Adhyashana

ï‚§80% of patients had a history of hotel food intake and Atidadhi Sevana

ï‚§ 43.33%, 40% and 30% were doing Atikshira Sevana, Atimamsa Sevana and Atiikshuvikara Sevana respectively

ï‚§73.33% of patients had a habit of overeating

ï‚§93.33% of patients were doing Avyavyama, 53.33% of patients had a history of Divasvapna and 93.33% of patients had a sedentary habit

ï‚§Sukha Shayyashana was found in 76.67% of patient

ï‚§60% of patients had Achinta while 46.67% had Harshanityata

ï‚§26.67% of patients had Bijadushti as a Nidana

Reviewing Dashavidha Pariksha-

ï‚§Half of the patients i.e.,50% had Vata- Kapha Prakriti while 56.67% of patients had Tamasika Manasa Prakriti

ï‚§100% of patients had Madhyama Sara, Samhanana and Pramana

ï‚§90% of patients were of Madhyama Satva

ï‚§93.33% of patients had Madhyama Satmya

ï‚§100% of patients belonged to Madhyama Vaya

ï‚§73.33% of patients had Pravara Abhyavaharana Shakti while Madhayama Jarana Shakti was detected in 76.67% of patients

ï‚§Avara Vyayama Shakti was noticed in 60% of patients

Reviewing of symptoms-

ï‚§100% of patients had Bhara Vriddhi and Gatra Gaurava. Chala Sphik Udara Stana was found in 83.33% of patients, Gatrasada in 86.67% of patients, Svedadhikya in 76.67% of patients and Kshudra Shvasa in 90% of patients. Among 30 patients of Sthaulya, 73.33% had Ati Kshudha, 36.67% had Ati Pipasa, 40% had Gatra Daurgandhya, 50% had Ayathopachaya, 46.67% had Ati Nindra, 63.33% had Snigdha Gatrata, 66.67% had Alpa Vyayama and 16.67% had Alpa Vyavaya.

ï‚§As the B.M.I increases patients falls into higher grade of obesity. In this study, 33.33% of patients, belonged to the range of 33.1-35.0 kg/m2

Effect of therapy

Group A (Virechana Karma)

ï‚§50%, 47.83%, 60.71%, 66.67% relief was found in Bhara Vriddhi, Chala Sphik Udara Stana, Gatra Gaurava and Gatra Daurgandhya respectively, with statistically highly significant result.

ï‚§46.15%, 58.33%, 63.64% relief was found in Ayathopachaya, Ati Pipasa and Snigdha Gatrata respectively, with statistically significant result.

ï‚§76.47%, 72.73%, 67.86%, 64%, 69.23% relief was found in Ati Kshudha, Kshudra Shvasa, Svedadhikya, Gatra Sada and Alpa Vyayama respectively, with statistically highly significant result.

ï‚§60% and 50% relief was found in Ati Nindra and Alpa Vyavaya respectively, but statistically insignificant result.

ï‚§Statistically highly significant (p=<0.0001) decrease in Body circumference measurements of mid arm, chest, waist, pelvis and mid-thigh by 3.90%, 3.15%, 5.85%, 4.17%, 2.63% respectively.

ï‚§Statistically highly significant (p=<0.0001) decrease in Skin fold thickness of biceps, triceps, subscapular and supra-illiac region by 12.82%, 7.46%, 6.25%, 13.69% respectively.

ï‚§8.47% improvement was found in Obesity on the basis of BMI, with statistically highly significant result (p=<0.0001). In this group, a maximum of 3.70 kg/m2 BMI decreased, minimum 1.70 kg/m2 BMI decreased.

ï‚§8.44% improvement was found in Obesity on the basis of Weight, with statistically highly significant result (p=<0.0001). In this group, a maximum of 11.0 kg weight was reduced and a minimum of 4.5 kg weight was reduced.

Group B (Shamana Snehapana)

ï‚§34.21%, 46.43%, 61.11%, 56.52%, 40.74%, 50% relief was found in Bhara Vriddhi, Gatra Gaurava, Ati Kshudha, Kshudra Shvasa, Gatra Sada and Alpa Vyayama respectively, with statistically highly significant result.

ï‚§11.54%, 8.33%, 40%, 23.08%, 41.67% relief was found in Chala Sphik Udara Stana, Ayathopachaya, Ati Pipasa, Svedadhikya and Ati Nindra respectively, but statistically in significant result.

ï‚§33.33% relief was found in Gatra Daurgandhya, Snigdha Gatrata and Alpa Vyavaya, but statistically insignificant result.

ï‚§Statistically highly significant decrease in Body circumference measurements of mid arm, chest, waist, pelvis and mid-thigh by 3.58%, 1.88%, 2.94%, 3.52%, 1.26% respectively.

ï‚§Statistically highly significant decrease in Skin fold thickness of biceps, triceps, subscapular and supra-illiac region by 6.60%, 2.99%, 2.44%, 8.28% respectively.

ï‚§3.55% improvement was found in Obesity on the basis of BMI, with statistically highly significant result (p=<0.0001). In this group, a maximum of 1.90 kg/m2 BMI decreased and a minimum of 0.60 kg/m2 BMI decreased.

ï‚§3.57% improvement was found in Obesity on the basis of Weight, with statistically highly significant result (p=<0.0001). In this group, a maximum of 5.3 kg weight was reduced and minimum 1.45 kg weight was reduced.

Total effect of therapy on Chief complaints

Group A (Virechana Karma)

ï‚§In this group, one patient (6.67%) showed complete remission and mild improvement, 3 patients (20%) showed marked improvement, 10 patients (66.67%) showed moderate improvement while none of the patient remained unchanged.

Group B (Shamana Snehapana)

ï‚§In this group, none of the patient showed complete or marked improvement, 3 patients (20%) showed moderate improvement, 11 patients (73.33%) showed mild improvement while one patient (6.67%) remained unchanged.

Comparison of therapy

ï‚§In Bhara Vriddhi and Chala Sphik Udara Stana statistically significant result was found in group A compared to group B. In Gatra Daurgandhya and Svedadhikya statistically highly significant result was found in group A compared to group B while in other complaints, a statistically significant difference was not found in improvement between group A and group B.

ï‚§In chest and waist circumference measurements statistically highly significant result was found in group A compared to group B. In mid-thigh circumference measurement statistically significant result was found in group A compared to group B while in other circumference measurements, a statistically significant difference was not found in improvement between group A and group B.

ï‚§In Skin fold thickness of four regions (biceps, triceps, subscapular, supra-illiac) there was statistically highly significant result in group A compared to group B.

ï‚§Among 30 patients of Sthaulya, 33.33% of patients were in the range of 0.6-1.2 kg/m2 of BMI decreased.

ï‚§Among 30 patients of Sthaulya, 36.67% were in the range of 1.0-3.0 kg weight reduction.

ï‚§In Obesity on the basis of BMI and Weight there was statistically highly significant result in group A compared to group B.

ï¶Discussion

The fourth section, titled Discussion, includes brief description of the conceptual study, logical and scientific interpretation of the observations gained, detailed analysis of the result and a possible mode of action of treatment based on Ayurvedic and modern parameters. An attempt was also made to postulate the disease’s Samprapti Ghataka, namely Kapha- Vata Dosha, Medadhatu as Dushya and Medovaha Srotodushti.

Inclusion Criteria
  • Patients between the age group of 18 to 60 years with classical signs and symptoms of Sthaulya.
  • Patients having BMI (Body Mass Index) greater than and equal to 25 and less than and equal to 35 will be taken for the study.
  • Virechana Yogya and Snehana Yogya as per Ayurvedic Classics will be included.
Exclusion Criteria
  • Patients having age less than 18 and greater than 60 will be excluded.
  • Known case of serious Cardiac, Renal or Hepatic diseases, AIDS, STD, TB, uncontrolled DM, Hypertension.
  • Obesity due to drugs e.g.,Anticonvulsant, Antipsychotics, Beta-blockers, long term use of steroids.
  • Pregnant and lactating women.
  • BMI less than 25 and greater than 35 will be excluded.
  • Virechana Ayogya and Snehana Ayogya patients as per Ayurvedic Classics will be excluded.
