Conditionsthis trial is conducted on mature first maxillary molars with the diagnosis of carious involvement of the pulp, with apical periodontitis, that show periapical change (radiographic PDL space widening, radiolucency) with/without Schneiderian membrane epithelium thickening on preoperative CBCT scan. Age range of patients to be included in the study is (18-65) years.CBCT, Operating Microscope, maxillary first molars, second mesiobuccal canal, Schneider membrane epithelium .

Inclusion Criteria
- •mature first maxillary molars with the diagnosis of carious involvement of the pulp, with acute or chronic apical periodontitis, that show periapical change (radiographic PDL space widening, radiolucency) with without Schneiderian membrane epithelium thickening on preoperative CBCT scan. Age range of patients to be included in the study is (18\-65\) years.
Exclusion Criteria
- •pregnancy, immunosuppression, patients received radiotherapy, uncontrolled diabetic patients, use of corticosteroids, patients taking bisphosphonates , unrestorable teeth , teeth with periodontal probing depths greater than 3 mm, uncontrolled periodontal disease, teeth with severely curved roots according to Shneider's classification, use of corticosteroids, root canal perforations if occur during endodontic treatment, separated instruments, vertical root fractures, signs of root resorption.
