1. Informal caregivers of adult patients- who were admitted to ICU with a neurological disease and are being planned for discharge
Minimum ICU stay of 3 days
May or may not have received ventilator support
Expected survival of at least 6 months after discharge from the hospital
2. Caregivers should -
Be be willing to give an informed consent.
Be willing to provide care to the patients after discharge.
Not have notable disability (defined as a Rankin score of 0-2) .
Possession of a working personal smartphone/tablet with either mobile cellular device or internet connectivity.
Patient with a premorbid dependency (mRS < 2) or a life expectancy <3 months
Caregivers with cognitive deficits or not willing to give consent
