1. Age > 18 years old and Follow up at Warfarin clinic of division of cardiothoracic surgery of Siriraj hospital.
2. Been using warfarin at least 3 months and INR stable around 2.00-3.00 at least 2 times.
3. Patient ≥ 60 years old with TMSE score > 23 or dementia with a care giver.
4. Patient or a care giver or relatives are able to use the application and having all-day internet
5. Patient or a care giver or relatives are able to understand and communicate with Thai language
6. Patient regularly follow up at warfarin clinic
1. Unable to sign the informed consent legally
2. Been diagnosed of any serious diseases, such as coronary artery disease without treatment, stroke without treatment, cancer, severe liver or renal disease
