1. Patients aged 20 and above.
2. Clinical diagnosis of diabetes taking oral medication and having HbA1c more than or equal 7.5 percent.
3. Clinical diagnosis of hypertension and taking oral medication and having blood pressure more than or equal to 140/90 mmHg.
4. Clinical diagnosis of renal disease and an estimated glomerular filtration rate (eGFR) between 30 - 59 ml/min/ 1.73 m2 calculated based on the CKD Epidemiology Collaboration (CKD EPI) formula.
5. Able to read, write, and speak in Thai language.
6. Able to access the internet and mobile applications.
1. Patients on dialysis or death.
2. Participating in other intervention.
3. Terminal illness other than CKD such as cancer or severe heart failure.
